Closed fractures that are not immediately stabilized?
Hello doctor, I accidentally fell last night and was diagnosed with a closed fracture of the palm after seeking medical attention.
The doctor originally scheduled me to go to the occupational therapy department this morning for immobilization, but it seems the doctor forgot that they are closed today.
I would like to ask if delaying the immobilization until tomorrow will have any impact on my injury.

Just don't move around too much; a difference of one day won't have a significant impact.

When it comes to closed hand fractures, timely intervention is crucial for optimal healing and recovery. The impact of delaying fixation, even for a day, can vary depending on several factors, including the type and location of the fracture, the degree of displacement, and the overall health of the patient.
In general, closed fractures are those where the skin remains intact, and there is no open wound associated with the fracture. The primary goal of treatment is to ensure proper alignment of the fractured bones, which is essential for healing and restoring function. Delaying fixation can lead to several potential complications:
1. Increased Swelling and Pain: After a fracture, it is common for the affected area to swell. Delaying fixation may exacerbate this swelling, leading to increased pain and discomfort. This can make it more challenging to achieve proper alignment during the eventual fixation.
2. Risk of Malunion or Nonunion: If the fracture is not stabilized promptly, there is a risk that the bones may heal in an improper position (malunion) or fail to heal altogether (nonunion). This can result in long-term functional impairment and may require more invasive treatments, such as surgery, to correct.
3. Joint Stiffness and Loss of Mobility: Prolonged immobilization of the hand can lead to stiffness in the joints and surrounding tissues. If fixation is delayed, the patient may experience a longer recovery time and may need additional physical therapy to regain full range of motion.
4. Increased Risk of Complications: Delaying treatment can increase the risk of complications, such as infection, especially if there are any associated soft tissue injuries. While closed fractures are less likely to be associated with infection than open fractures, any delay in treatment can still pose risks.
5. Psychological Impact: The uncertainty and anxiety associated with a delay in treatment can also affect the patient's mental well-being. Patients may worry about the implications of the delay on their recovery and future function.
In your case, while a one-day delay in fixation may not lead to significant complications, it is still advisable to seek treatment as soon as possible. If you are experiencing increased pain, swelling, or any other concerning symptoms, it is essential to communicate this to your healthcare provider. They may be able to provide guidance on managing your symptoms until you can receive the necessary treatment.
In summary, while a short delay in fixation may not drastically alter the outcome for every patient, it is always best to follow the recommended treatment plan as closely as possible to ensure optimal healing and recovery. If you have any concerns or questions about your specific situation, do not hesitate to reach out to your healthcare provider for further advice. They can provide personalized recommendations based on the specifics of your fracture and overall health.
