Pain beneath the scrotum?

I have a painful area under my left scrotum (a little above where the scrotum connects).
The skin at the bottom of the left scrotum near that junction hurts when pressed, but it doesn't hurt at all during normal activities.
It only hurts when I press on it with my hand.
Additionally, there is a dark spot in that area.
What could this be? Thank you.

If there is tenderness in the scrotum, it may be due to orchitis or epididymitis.
If the pain is closer to the surface, it could be caused by a sebaceous cyst or folliculitis.
It is recommended to visit a clinic for a thorough examination to confirm the cause.
Online consultations cannot replace an in-person evaluation and physical examination by a physician.
If there is a medical condition, it is still advisable to seek care at a healthcare facility.

Pain under the left scrotum can be concerning, and it's important to consider various potential causes. Based on your description, the pain occurs only when pressure is applied, and you also mention a black spot in the area. Here are some possible explanations for your symptoms:
1. Inguinal Hernia: One common cause of pain in the scrotal area is an inguinal hernia. This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. It can cause discomfort, especially when pressure is applied, and may be associated with a bulge in the groin or scrotum.

2. Epididymitis: This is an inflammation of the epididymis, which is the tube that carries sperm from the testicle. Epididymitis can cause pain and swelling in the scrotum and may be related to infection or injury. While it usually presents with more persistent pain, localized tenderness can occur.

3. Testicular Torsion: Although this condition typically presents with sudden and severe pain, it’s worth mentioning. Testicular torsion occurs when the spermatic cord becomes twisted, cutting off the blood supply to the testicle. This is a medical emergency and requires immediate attention.

4. Varicocele: This refers to enlarged veins within the scrotum, similar to varicose veins. A varicocele can cause a dull ache or discomfort, particularly when standing or during physical activity. It may not always be painful but can lead to a feeling of heaviness in the scrotum.

5. Skin Conditions: The black spot you mentioned could be a benign skin condition, such as a mole or a seborrheic keratosis. However, it could also be a sign of a more serious condition, such as a dermatological issue or even a skin infection. If the spot changes in appearance, it should be evaluated by a healthcare provider.

6. Trauma or Injury: If you have had any recent trauma to the area, even minor, it could lead to localized pain and bruising. This could explain the tenderness when pressure is applied.

7. Referred Pain: Sometimes, pain in the scrotal area can be referred from other areas, such as the lower abdomen or pelvis. Conditions affecting the kidneys or urinary tract, such as kidney stones, can sometimes present with scrotal pain.

Given that the pain is only present when pressure is applied and that you have a black spot, it is advisable to consult a healthcare provider for a thorough examination. They may perform a physical examination and possibly imaging studies, such as an ultrasound, to assess the area more accurately.
In summary, while there are several potential causes for your symptoms, the best course of action is to seek medical evaluation to rule out any serious conditions and to receive appropriate treatment if necessary. Early diagnosis and management can help prevent complications and ensure better outcomes.

Visit the hospital for a physician evaluation, as it may be a case of varicocele. This condition can cause significant discomfort, increase scrotal temperature, and affect sperm quality. A semen analysis can be performed.
