Prepuce issues

Hello doctor, I have phimosis.
When I wash during a shower, I notice that the smegma seems to increase slightly, and the inner side of the foreskin is a bit red, although the redness is not very noticeable.
The inner foreskin feels slightly itchy.
After I clean the smegma during my shower, the itching decreases, but after about 5 to 6 hours, the smegma builds up again and the itching returns, although it is tolerable.
I'm not sure if the smegma is causing the itching on the inner foreskin.
I have not engaged in sexual activity, and this condition has been ongoing for two weeks (the first 2 to 3 days when it started were very itchy, but since then I have been cleaning the smegma twice daily, and it has improved significantly).
What could be wrong with me?

More Info

Phimosis is a condition characterized by the inability to retract the foreskin over the glans (the head of the penis). It can be a congenital condition or develop over time due to various factors, including inflammation, scarring, or infection. Understanding the causes, symptoms, and treatment options for phimosis is essential for managing the condition effectively.

Causes of Phimosis
1. Congenital Factors: Many males are born with a tight foreskin that cannot be retracted. This is common in infants and young children, and in many cases, the foreskin becomes retractable naturally as the child grows.

2. Inflammation and Infection: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can lead to scarring and tightening of the foreskin. Poor hygiene can contribute to the buildup of smegma, a substance that can irritate the skin and lead to inflammation.

3. Scarring: Trauma or injury to the foreskin can result in scarring, which may lead to phimosis. This can occur from aggressive cleaning, infections, or other physical trauma.

4. Skin Conditions: Certain skin conditions, such as psoriasis or eczema, can affect the genital area, leading to phimosis.

Symptoms of Phimosis
- Inability to Retract the Foreskin: The primary symptom is the inability to pull back the foreskin over the glans.

- Discomfort or Pain: Some individuals may experience discomfort or pain, especially during erections.

- Redness and Itching: As you described, the inner foreskin may become red and itchy, particularly if there is a buildup of smegma or if there is an underlying infection.

- Swelling: In some cases, the foreskin may appear swollen or inflamed.

Treatment Options
1. Improved Hygiene: Regular cleaning of the genital area can help reduce the buildup of smegma and prevent irritation. It is essential to gently wash the area with mild soap and water.

2. Topical Treatments: If there is inflammation or infection, a healthcare provider may prescribe topical corticosteroids to reduce inflammation and promote the retraction of the foreskin. These treatments can help soften the skin and make it easier to retract.

3. Stretching Exercises: Gradual stretching of the foreskin can sometimes help alleviate phimosis. This should be done gently and under the guidance of a healthcare provider to avoid injury.

4. Surgical Options: In cases where phimosis is severe or does not respond to conservative treatments, surgical options such as circumcision (removal of the foreskin) or preputioplasty (surgical widening of the foreskin) may be considered.

When to Seek Medical Attention
Given your symptoms of increased smegma, slight redness, and itching, it is advisable to consult a healthcare provider. They can assess your condition, rule out any infections, and recommend appropriate treatment. It is important to address these symptoms early to prevent complications such as recurrent infections or scarring.

In summary, phimosis can be managed effectively with proper hygiene, topical treatments, and, if necessary, surgical interventions. Your symptoms suggest that there may be an underlying issue that needs to be addressed, so seeking medical advice is a prudent step.
