Post-operative issues after circumcision?

Hello Doctor, I recently underwent a circumcision procedure and would like to ask a few questions.
1.
The edge of the glans appears to be white; what could this condition be? 2.
When bending over, the glans tends to retract into the foreskin; is this normal in the days following the surgery? Thank you, Doctor.

H, 20~29 year old female. Ask Date: 2018/04/01

Dr. Du Shixi reply Urology

Hello: 1) You can rinse with saline solution; it may be debris.
2) The wound will heal in about 1-2 weeks, and once the swelling subsides, you will be able to see the true length of the foreskin.
Wishing you good health.

After undergoing circumcision, it is common for patients to have concerns regarding the healing process and any unusual symptoms they may experience. Let's address your questions one by one.

1. White Appearance of the Glans: It is not uncommon for the glans (the head of the penis) to appear white or pale after circumcision. This can be due to several factors, including the healing process itself. After the removal of the foreskin, the glans is exposed for the first time and may undergo a process called keratinization, where the skin thickens and becomes less sensitive. This can give it a whitish appearance. Additionally, if there is any swelling or irritation from the surgery, it may also contribute to this appearance. As healing progresses, the color and texture of the glans should return to normal. However, if you notice any signs of infection, such as increased redness, swelling, discharge, or persistent pain, it is important to consult your healthcare provider.

2. Retracting of the Glans When Bending: It is also normal for the glans to retract into the remaining skin when bending or moving in certain ways, especially in the early days following the surgery. This is due to the sensitivity and the adjustment of the tissues after the circumcision. The area is still healing, and the muscles and skin may not yet be accustomed to the new configuration. As the healing progresses, you should find that this becomes less of an issue. However, if you experience significant discomfort or if the glans retracts excessively, it would be wise to discuss this with your surgeon or a urologist.

Additional Considerations for Post-Circumcision Care:
- Hygiene: Keeping the area clean is crucial. Gently wash the area with mild soap and water, and avoid any harsh chemicals or fragrances that could irritate the healing skin.

- Moisturization: Some doctors recommend applying a thin layer of antibiotic ointment to the area to prevent infection and keep the skin moist, but this should be done as per your doctor's instructions.

- Pain Management: It is normal to experience some discomfort after the procedure. Over-the-counter pain relievers, such as acetaminophen or ibuprofen, can help manage pain. Always follow the dosage instructions provided by your healthcare provider.

- Activity Restrictions: Avoid strenuous activities, heavy lifting, or sexual activity until your doctor gives you the go-ahead. This is to ensure proper healing and to prevent complications.

- Follow-Up Appointments: Make sure to attend any follow-up appointments with your healthcare provider to monitor your healing process and address any concerns you may have.

When to Seek Medical Attention:
- If you notice any signs of infection (increased redness, swelling, discharge, or fever).

- If you experience severe pain that is not relieved by over-the-counter medications.

- If the white appearance of the glans persists beyond a few weeks or worsens.

- If you have any other concerns that arise during your recovery.

In conclusion, while some changes and sensations after circumcision can be concerning, they are often part of the normal healing process. However, staying vigilant and maintaining open communication with your healthcare provider is key to ensuring a smooth recovery. If you have any further questions or concerns, do not hesitate to reach out to your doctor for personalized advice.
