Premature ejaculation
Hello, Dr.
Hsu.
I haven't engaged in sexual intercourse for about six months, and recently I started having sexual activity again.
However, as soon as penetration occurs, I feel an urge to ejaculate.
Initially, I thought this was due to the long break from sexual activity, but after several attempts, I found that the situation remains the same or has even worsened.
I even tried to masturbate beforehand to reduce sensitivity, but during intercourse, I still ejaculate quickly, regardless of whether I am erect or not.
I feel an overwhelming urge to ejaculate anytime I have any contact with my penis, as if I am about to explode.
I have been exercising recently, but the situation has not improved, and my erections do not feel as firm as they used to.
Sometimes I experience morning erections, and sometimes I do not.

More Info
Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by the inability to control ejaculation during sexual intercourse, leading to ejaculation that occurs sooner than desired. Understanding the causes and potential solutions for PE is crucial for those experiencing this issue.
Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, stress, and relationship issues can significantly impact sexual performance. The pressure to perform can lead to heightened sensitivity and a quickened response during sexual activity. This is particularly common in men who have not engaged in sexual activity for an extended period, as they may feel increased pressure to perform.
2. Biological Factors: Hormonal imbalances, neurotransmitter levels, and certain medical conditions can contribute to PE. For instance, low levels of serotonin, a neurotransmitter that plays a role in mood and sexual function, have been linked to premature ejaculation. Additionally, conditions such as prostatitis or thyroid issues can also affect sexual function.
3. Physical Factors: Certain physical conditions, such as pelvic floor muscle dysfunction or hypersensitivity of the penis, can lead to premature ejaculation. Men who have experienced sexual trauma or have a history of sexual dysfunction may also be at a higher risk.
4. Lifestyle Factors: Substance abuse, including alcohol and recreational drugs, can impair sexual function. Poor physical health, lack of exercise, and obesity can also contribute to sexual dysfunction, including PE.
Solutions and Management Strategies
1. Behavioral Techniques: One of the most effective methods for managing PE is through behavioral techniques. The "stop-start" method involves pausing sexual stimulation before ejaculation and resuming once the urge has subsided. This can help increase control over ejaculation. Another technique is the "squeeze" method, where pressure is applied to the base of the penis to delay ejaculation.
2. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises, commonly known as Kegel exercises, can improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles used to stop urination.
3. Desensitizing Products: Over-the-counter desensitizing creams or sprays can help reduce sensitivity and prolong the time before ejaculation. However, it is essential to use these products carefully to avoid affecting your partner's experience.
4. Counseling and Therapy: If psychological factors are contributing to PE, seeking the help of a therapist or counselor can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and relationship issues that may be impacting sexual performance.
5. Medical Treatment: In some cases, medication may be prescribed to help manage PE. Selective serotonin reuptake inhibitors (SSRIs), commonly used as antidepressants, have been found to delay ejaculation as a side effect. Other medications, such as topical anesthetics, may also be considered.
6. Lifestyle Changes: Improving overall health through regular exercise, a balanced diet, and reducing alcohol and drug use can positively impact sexual function. Additionally, practicing relaxation techniques such as mindfulness or yoga can help reduce anxiety related to sexual performance.
Conclusion
Premature ejaculation can be a distressing condition, but it is important to remember that it is a common issue and can often be managed effectively. If you are experiencing persistent problems with PE, it is advisable to consult with a healthcare professional who can provide personalized advice and treatment options. Open communication with your partner about your experiences and concerns can also help alleviate anxiety and improve intimacy. Remember, addressing the issue early can lead to better outcomes and improved sexual satisfaction for both partners.
