Urology questions?
Hello Doctor: I have the following questions I would like to ask: 1.
I have been engaging in sexual activity since childhood, and now at 37 years old, my genitals are still small.
Both my testicles and penis are small, with the penis reaching a maximum length of 5 centimeters when erect.
However, I have undergone semen analysis and the doctor said it was normal, but I am still infertile.
I am unsure of the reason.
Does starting sexual activity at a young age affect the size of the penis later in life? If I practice appropriate abstinence now, will my penis grow larger? I was once told by a nurse during a colonoscopy that my size was quite small (as it was visible).
2.
Additionally, sometimes when I masturbate, I experience pain in the area below the testicles (the area beneath the testicles) about 4 out of 5 times after masturbation.
Is this a medical issue, or could it be related to undescended testicles?

Dr. Fu Shizhang reply Urology
1.
The size of the penis is not related to the onset of sexual activity and may be associated with genetic or chromosomal abnormalities.
However, if a semen analysis is normal, male infertility can generally be ruled out.
2.
This phenomenon is not related to cryptorchidism; if cryptorchidism is suspected, it is recommended to consult a urologist.
Online consultations cannot replace an in-person medical interview and physical examination by a physician.
If there is a medical condition, it is still advisable to seek care at a healthcare facility.

More Info
Your concerns regarding urology, particularly about size, fertility, and pain, are quite common, and it's important to address each aspect carefully.
1. Size and Sexual Activity: You mentioned that you have been sexually active since a young age and are now 37 years old, with a penis size of 5 cm when erect. It's crucial to understand that penis size can vary significantly among individuals and is influenced by genetic factors rather than sexual activity. Engaging in sexual activity from a young age does not typically affect the size of the penis. The notion that abstaining from sexual activity could lead to an increase in size is a myth; penis size is largely determined by genetics and hormonal influences during puberty. If you have concerns about your size, it may be beneficial to speak with a urologist who can provide guidance and reassurance.
2. Fertility Concerns: You mentioned that you have undergone semen analysis, and the results were normal, which is a positive sign regarding your fertility. Normal semen analysis indicates that your sperm count, motility, and morphology are within acceptable ranges. If you are still facing challenges with conception, it may be worthwhile to explore other factors that could affect fertility, such as the health of your partner, timing of intercourse, and overall reproductive health. Consulting a fertility specialist could provide additional insights and options.
3. Pain During Masturbation: Experiencing pain in the area below the testicles after masturbation is not uncommon, but it should not be ignored. This discomfort could be due to several factors, including:
- Overstimulation: Frequent masturbation can lead to soreness or discomfort in the genital area.
- Epididymitis: This is an inflammation of the epididymis, which can cause pain in the testicular area. It can be caused by infections or other underlying conditions.
- Inguinal Hernia: Sometimes, pain in the groin or testicular area can be related to a hernia, which occurs when tissue pushes through a weak spot in the abdominal muscles.
- Testicular Torsion: Although less common, this is a serious condition that requires immediate medical attention. It occurs when the spermatic cord becomes twisted, cutting off blood supply to the testicle.
If the pain persists or worsens, it is essential to consult a healthcare professional for a thorough examination. They may perform a physical examination and possibly imaging studies to determine the cause of your discomfort.
4. Hidden Testes (Cryptorchidism): You mentioned the possibility of having undescended testes (cryptorchidism). This condition occurs when one or both testicles fail to move into the scrotum before birth. If you suspect this condition, it is crucial to seek medical advice, as undescended testes can lead to complications such as infertility and increased risk of testicular cancer later in life.
In summary, while concerns about size, fertility, and pain are valid, it is essential to approach them with accurate information and professional guidance. Consulting with a urologist can provide you with personalized advice and treatment options tailored to your specific situation. Remember, open communication with healthcare providers is key to addressing any health concerns effectively.
