Medication consultation

I take blood pressure medication (Cozaar) daily.
Is this medication classified as a nitrate? Can I also take Viagra? Will there be any interactions or side effects between the two medications? Thank you!

Tianren, 60~69 year old female. Ask Date: 2018/04/26

Dr. Chen Bozhang reply Family Medicine

Dear Mr.
Tianren: The antihypertensive medication COZAAR (Losartan) is not classified as a nitrate, and therefore does not interact with Viagra (Sildenafil).
However, since you are a hypertension patient, it is crucial to be aware of the following contraindications when using Viagra: the presence of severe cardiovascular diseases, such as heart failure and unstable angina.
Additionally, if your blood pressure is poorly controlled (above 170/110 mmHg), or if you have recently experienced a stroke or myocardial infarction, you should avoid using Viagra to prevent unpredictable cardiovascular risks.
Sincerely, Dr.
Chen Bozhang, Department of Family Medicine, Keelung Hospital, Ministry of Health and Welfare.

More Info

Cozaar, known generically as Losartan, is an angiotensin II receptor blocker (ARB) primarily used to treat high blood pressure and protect kidney function in patients with diabetes. It is not classified as a nitrate or nitroglycerin, which are commonly used to treat angina and heart-related conditions. Therefore, it does not share the same mechanism of action as these medications.

When considering the use of Viagra (sildenafil) alongside Cozaar, it is important to note that there are no significant drug interactions reported between these two medications. Viagra works by increasing blood flow to the penis, facilitating an erection, and it does this by inhibiting the enzyme phosphodiesterase type 5 (PDE5). Cozaar, on the other hand, helps to lower blood pressure by blocking the effects of angiotensin II, a substance in the body that causes blood vessels to constrict.
While there is no direct interaction between Cozaar and Viagra, caution is advised when combining any medications. The primary concern arises when Viagra is used in conjunction with nitrates (like nitroglycerin), as this combination can lead to a dangerous drop in blood pressure. Since Cozaar is not a nitrate, this specific risk does not apply. However, it is always prudent to consult with your healthcare provider before starting any new medication, including Viagra, especially if you have underlying health conditions or are taking multiple medications.

Regarding side effects, both Cozaar and Viagra have their own profiles. Common side effects of Cozaar may include dizziness, fatigue, and potential kidney function changes, particularly in patients with pre-existing kidney issues. Viagra can cause headaches, flushing, nasal congestion, and in some cases, visual disturbances. While these side effects are generally mild, they can vary from person to person.

In your case, since you are already on Cozaar for blood pressure management, it is essential to monitor your blood pressure regularly, especially when introducing a new medication like Viagra. If you experience any unusual symptoms, such as significant dizziness, fainting, or prolonged erections (lasting more than four hours), you should seek medical attention immediately.

In summary, Cozaar is not a nitrate, and it does not interact negatively with Viagra. However, it is crucial to have a thorough discussion with your healthcare provider to ensure that this combination is safe for you, considering your overall health and any other medications you may be taking. Always prioritize open communication with your healthcare team to manage your medications effectively and safely.
