Itching of the glans and anus?
Hello, about half a month ago, my girlfriend and I traveled to Japan, and it was our first time having sexual intercourse.
During that time, we had several instances of unprotected sex and also soaked in hot springs multiple times.
A few days later, I experienced itching in the glans and anus, but there were no visible abnormalities.
However, in the past few days, I have occasionally noticed a clear discharge from the urethral opening.
What could this problem be? Additionally, since I reside in Southeast Asia, I face language barriers when visiting doctors.
What kind of medications can I purchase at a pharmacy to manage this issue? Thank you!

Hello: This facility cannot provide medication references, and we apologize for that.
However, in your situation, it is important to pay attention to safe sexual practices.
It is advisable to limit hot spring baths.
If you experience itching in the glans or anus without any skin abnormalities, you can take a shower but avoid hot water.
Do not use soap or body wash in those areas; instead, clean with plain water and dry thoroughly.
Wear loose and breathable underwear.
As for the clear fluid, it may be prostatic fluid.
If you have no symptoms while urinating, drink plenty of water, avoid irritating foods and alcohol, and just monitor the situation.
If you experience pain during urination or notice pus, you should seek medical attention.
Wishing you good health.

More Info
Itching in the genital area, particularly in the penis and anus, can be a distressing experience and may arise from various causes. Given your recent sexual activity and the symptoms you've described, it is important to consider several potential factors that could be contributing to your discomfort.
Possible Causes
1. Sexually Transmitted Infections (STIs): Engaging in unprotected sex can increase the risk of STIs, which may present with symptoms such as itching, discharge, or irritation. Common STIs that could cause these symptoms include gonorrhea, chlamydia, and herpes. The transparent discharge you mentioned could be indicative of urethritis, which is inflammation of the urethra often caused by infections.
2. Fungal Infections: Yeast infections can occur in both men and women and may lead to itching and irritation in the genital area. These infections are often characterized by a thick, white discharge, but they can also present with itching without significant discharge.
3. Bacterial Infections: Conditions such as bacterial vaginosis in female partners can lead to symptoms in male partners, including itching and discharge.
4. Irritation or Allergic Reactions: Sometimes, irritation can stem from soaps, lotions, or even the materials in condoms or clothing. Allergic reactions to personal care products can also cause localized itching.
5. Skin Conditions: Conditions such as eczema or psoriasis can affect the genital area, leading to itching and discomfort.
Recommended Actions
1. Consult a Healthcare Provider: It is crucial to see a healthcare provider for an accurate diagnosis. They may perform tests to check for STIs or other infections. Given the language barrier you mentioned, consider bringing a friend who speaks the local language or using translation apps to facilitate communication.
2. Over-the-Counter Treatments: While it is important to identify the underlying cause, some over-the-counter treatments may provide temporary relief:
- Antifungal Creams: If a yeast infection is suspected, antifungal creams such as clotrimazole or miconazole can be effective.
- Hydrocortisone Cream: This can help reduce inflammation and itching but should be used sparingly and not on broken skin.
- Antihistamines: Oral antihistamines may help if the itching is due to an allergic reaction.
3. Avoid Irritants: Until you can see a doctor, avoid using scented soaps, lotions, or other products in the genital area. Wearing loose-fitting, breathable underwear can also help reduce irritation.
4. Practice Safe Sex: To prevent future occurrences, consider using condoms consistently, even with new partners, to reduce the risk of STIs.
Conclusion
While over-the-counter treatments may provide some relief, they are not a substitute for professional medical advice. The symptoms you are experiencing could be indicative of an infection or another underlying condition that requires specific treatment. It is advisable to seek medical attention as soon as possible to ensure proper diagnosis and treatment. In the meantime, maintaining good hygiene and avoiding potential irritants can help manage your symptoms.
