Itchy and red penis?

Doctor, I initially used body lotion for lubrication during masturbation, and I’m not sure if that caused my glans to become red.
There is a sore and redness in the groove connecting the glans to the shaft, and it also itches.
Sometimes, after washing the area the next day, I notice a white substance, but I’m afraid to scrub too hard because it might cause more irritation.
Even after washing, it still feels itchy and red, and this has been going on for about 3-4 days.
Could you please help me understand what this condition might be? If I prefer not to visit a large hospital, are there any medications available at a local pharmacy that could help? Thank you!

A-Lin, 20~29 year old female. Ask Date: 2010/02/18

Dr. Liu Jianting reply Urology

It is possible that the balanoposthitis is caused by a fungal infection.
Please visit a hospital to consult a urologist for examination and treatment.

The symptoms you are experiencing, including redness, itching, and the presence of a white substance on the penis, could be indicative of several conditions, most commonly a form of dermatitis or an infection such as candidiasis (yeast infection). Here’s a breakdown of potential causes and remedies based on your description.

Possible Causes:
1. Irritant Contact Dermatitis: This condition can occur when the skin reacts to irritants, such as lotions or soaps that are not specifically designed for sensitive areas. The use of body lotion for masturbation may have caused irritation, leading to redness and itching.

2. Candidiasis (Yeast Infection): The symptoms you describe, particularly the white discharge, could suggest a yeast infection. This is more common in warm, moist areas and can occur if the balance of normal flora is disrupted.

3. Balanitis: This is an inflammation of the glans (the head of the penis) and can be caused by irritants, infections, or poor hygiene. Symptoms typically include redness, itching, and sometimes discharge.

4. Sexually Transmitted Infections (STIs): While less likely given your description, certain STIs can also cause similar symptoms. If you have had unprotected sexual contact, this should be considered.

Remedies and Treatment:
1. Avoid Irritants: Stop using the body lotion for lubrication. Instead, consider using water-based lubricants that are specifically designed for sexual activity. These are less likely to cause irritation.

2. Gentle Hygiene: Clean the area gently with mild soap and water. Avoid scrubbing, as this can exacerbate irritation. Pat the area dry instead of rubbing.

3. Topical Treatments: Over-the-counter antifungal creams (like clotrimazole or miconazole) can be effective if a yeast infection is suspected. Hydrocortisone cream may help reduce inflammation and itching, but it should be used sparingly and not for prolonged periods.

4. Avoid Moisture: Keep the area dry and avoid tight clothing that can trap moisture. Consider wearing breathable cotton underwear.

5. Consult a Pharmacist: If you prefer not to visit a doctor, a pharmacist can recommend over-the-counter treatments suitable for your symptoms. They can provide guidance on the appropriate products to use.

6. Seek Medical Attention: If symptoms persist for more than a few days, worsen, or if you notice any unusual discharge, it is important to consult a healthcare professional. They can provide a proper diagnosis and treatment plan.

When to See a Doctor:
- If the redness and itching do not improve with home treatments within a few days.

- If you develop additional symptoms such as fever, increased pain, or unusual discharge.

- If you have concerns about STIs or if you have had unprotected sexual contact.

Conclusion:
While it can be uncomfortable and concerning to experience these symptoms, many cases of redness and itching can be resolved with proper care and treatment. Avoiding irritants, maintaining good hygiene, and using appropriate topical treatments can often alleviate the symptoms. However, do not hesitate to seek professional medical advice if your condition does not improve, as a healthcare provider can offer the most accurate diagnosis and effective treatment options.
