Phimosis issue
The foreskin covering the glans has recently become red, swollen, and painful near the urinary opening.
I usually pull back the foreskin to wash the glans and the coronal sulcus while showering.
I visited a urologist, and my urinalysis came back normal.
Upon physical examination, no abnormalities were found on the glans, and I was prescribed a topical ointment, Topsym Cream.
Could the redness, swelling, and pain be due to balanitis, or could it be another condition?

Dr. Du Shixi reply Urology
Hello: If there has been no unsafe sexual behavior, the redness and pain are mostly caused by irritation or heat, which is likely balanitis.
Maintaining cleanliness is sufficient, or applying ointment can help improve the condition.
However, if there has been unsafe sexual behavior, there may be a risk of a more specific infection, and medical treatment should be sought.
Wishing you good health.

More Info
Phimosis is a condition in which the foreskin of the penis cannot be easily retracted over the glans (the head of the penis). This condition can lead to various symptoms and complications, including inflammation, pain, and difficulty with urination. Understanding the causes, symptoms, and treatment options for phimosis is essential for effective management.
Causes of Phimosis
Phimosis can be classified into two types: physiological and pathological.
1. Physiological Phimosis: This is common in infants and young boys, where the foreskin is naturally non-retractable due to the way it is attached to the glans. Most boys will outgrow this condition as they reach puberty.
2. Pathological Phimosis: This occurs due to scarring or inflammation of the foreskin, often resulting from conditions such as:
- Balanitis: Inflammation of the glans, which can be caused by infections, poor hygiene, or irritants.
- Balanoposthitis: Inflammation of both the glans and the foreskin.
- Infections: Fungal or bacterial infections can lead to swelling and pain.
- Skin Conditions: Conditions like psoriasis or lichen sclerosus can also cause scarring and phimosis.
Symptoms of Phimosis
The symptoms of phimosis can vary but may include:
- Difficulty retracting the foreskin
- Pain or discomfort during urination or sexual activity
- Swelling or redness of the foreskin or glans
- Discharge or unusual odor
- Recurrent urinary tract infections (UTIs)
In your case, the redness and swelling near the urinary opening, along with pain, could indeed suggest a condition like balanitis or balanoposthitis, especially if you have been experiencing discomfort and have a history of phimosis.
Treatment Options
The treatment for phimosis depends on the severity of the condition and the underlying cause. Here are some common approaches:
1. Topical Treatments: As you mentioned, your doctor prescribed a topical cream (Topsym Cream) which may contain corticosteroids to reduce inflammation and help with retraction of the foreskin. This is often the first line of treatment for mild cases.
2. Hygiene Practices: Maintaining proper hygiene is crucial. Gently cleaning the area and avoiding irritants can help reduce inflammation and prevent infections.
3. Stretching Exercises: Some healthcare providers recommend gentle stretching exercises of the foreskin to gradually increase its elasticity.
4. Circumcision: In cases where phimosis is severe or recurrent infections occur, circumcision (surgical removal of the foreskin) may be recommended. This is often considered when conservative treatments fail.
5. Consultation with a Specialist: If symptoms persist or worsen, it may be beneficial to consult a urologist for further evaluation and management.
Conclusion
In summary, the symptoms you are experiencing could indeed be indicative of a condition related to phimosis, such as balanitis or balanoposthitis. It is essential to follow up with your healthcare provider if the symptoms do not improve with topical treatment or if they worsen. They may recommend further diagnostic tests or a referral to a specialist for more comprehensive care. Remember, maintaining good hygiene and following your doctor's advice is key to managing this condition effectively.
