Irregular menstruation! Is there a possibility of pregnancy?

Hello doctor, on May 9th, I had unprotected sexual intercourse with my girlfriend, but there was no ejaculation.
However, there may have been pre-ejaculate fluid involved.
This was three days after her ovulation period (according to the app, the likelihood of pregnancy is low).
Her app indicated that her period was due on May 19th, but a few days earlier, on May 17th, she noticed a small amount of brown discharge along with blood clots and secretions (only once).
She has also experienced lower abdominal pain and lower back pain.
The back and abdominal pain lasted only a few hours, but as of May 20th, her period still has not arrived.
She has slight breast tenderness and pain.
My girlfriend generally has a cold body constitution, has irregular eating habits with a very small appetite, and usually goes to bed by 10:30 PM.
Her menstrual cycle is irregular.
Could there be a significant possibility of pregnancy, or could these symptoms be due to other health issues?

Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
Additionally, early pregnancy does not always present specific symptoms, and subjective symptoms cannot be used to make a determination.
It is recommended that if your period is more than a week late or if you are uncertain whether the bleeding is menstrual, you should check for pregnancy or seek assistance from a physician.
Thank you! Wishing you peace and health, Dr.
Chang.

More Info

Irregular menstruation can indeed indicate pregnancy, but it can also be a sign of other health issues. In your case, since your girlfriend had unprotected intercourse during her ovulation period, there is a possibility of pregnancy, especially if there was any pre-ejaculatory fluid involved. The timing of her menstrual cycle is also a crucial factor to consider.
Typically, a woman's menstrual cycle ranges from 21 to 35 days, with an average of 28 days. If her period was expected on May 19 and she has not yet received it, it could be an indication of pregnancy. The presence of brown discharge and blood clots a couple of days before her expected period could suggest implantation bleeding, which occurs when a fertilized egg attaches to the lining of the uterus. This can happen around the time a woman would expect her period, leading to confusion.

The symptoms your girlfriend is experiencing, such as lower abdominal pain, back pain, and breast tenderness, can also be associated with premenstrual syndrome (PMS) or early pregnancy. However, these symptoms alone are not definitive indicators of pregnancy. The absence of her period, combined with these symptoms, does warrant a pregnancy test for confirmation.
It’s also important to consider other health issues that could cause irregular menstruation. Factors such as stress, significant weight changes, hormonal imbalances, and underlying medical conditions like polycystic ovary syndrome (PCOS) can affect menstrual regularity. Since you mentioned that your girlfriend has a "cold" body constitution and irregular eating habits, these factors could also contribute to her menstrual irregularities.
If she continues to experience irregularities in her cycle or if the symptoms worsen, it would be advisable for her to consult a healthcare professional. A doctor can perform a thorough evaluation, including a physical examination and possibly blood tests, to determine if there are any underlying health issues that need to be addressed.
In summary, while there is a possibility of pregnancy given the circumstances, irregular menstruation can also be influenced by various health factors. A home pregnancy test can provide initial insight, but consulting a healthcare provider for a comprehensive assessment is the best course of action to ensure both her health and well-being.
