Can I get pregnant?

Hello doctor, I have been taking oral contraceptives for over three months now, and I am on a 21-pill regimen.
I have taken them consistently without missing any doses.
My menstrual cycle is 28 days, and my last period started on May 30.
I had sexual intercourse on June 5, and on June 7, I took anti-inflammatory medication and antibiotics due to feeling unwell.
Since then, I have not had any sexual intercourse.
Will this affect the effectiveness of the oral contraceptive? Is there a chance of pregnancy? I have been feeling bloated and uncomfortable in my abdomen lately.
Please help me with this, thank you.

Ms. Chen, 20~29 year old female. Ask Date: 2018/06/14

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: Some medications can indeed affect the efficacy of birth control pills, such as certain antibiotics, enzyme inducers, and some HIV treatment medications.
Among these, cold medications often contain antibiotics, so if you need to take any of the aforementioned medications while on birth control, it is advisable to use additional contraceptive measures for seven days.
Additionally, pregnancy cannot be determined solely based on subjective symptoms.
It is recommended that if your period is more than a week late, you should take a urine test to check for pregnancy.
Thank you! Wishing you peace and health, Dr.
Zhang.

When it comes to the effectiveness of birth control pills, it's important to understand how they work and the factors that can influence their efficacy. Birth control pills, particularly combination pills that contain both estrogen and progestin, are designed to prevent ovulation, thicken cervical mucus, and thin the uterine lining, making it difficult for sperm to fertilize an egg and for a fertilized egg to implant.

In your case, you have been taking the pill consistently for over three months without missing any doses, which is crucial for maintaining its effectiveness. Generally, when taken correctly, birth control pills are over 99% effective at preventing pregnancy. However, certain factors can affect their efficacy, including:
1. Antibiotics: While most antibiotics do not interfere with the effectiveness of birth control pills, there are exceptions. The antibiotic rifampin is known to reduce the effectiveness of hormonal contraceptives. If you were prescribed a different antibiotic, it is unlikely to have affected your birth control. However, it's always best to consult with your healthcare provider about any medications you are taking.

2. Gastrointestinal Issues: If you experienced vomiting or severe diarrhea after taking the pill, this could potentially reduce its effectiveness, as the body may not absorb the hormones properly. However, if you did not have these issues, your birth control should still be effective.

3. Timing and Consistency: Since you mentioned that you have been taking your pills on time, this is a positive factor in maintaining their effectiveness. Consistency is key when it comes to oral contraceptives.

Regarding your concerns about pregnancy, if you have been taking your pills correctly and have not experienced any significant issues that could compromise their effectiveness, the likelihood of becoming pregnant is very low. However, if you are experiencing symptoms such as abdominal bloating or discomfort, it could be related to other factors, such as hormonal changes, digestive issues, or even stress.

If you are still concerned about the possibility of pregnancy, consider taking a home pregnancy test for peace of mind. These tests are generally accurate if taken after a missed period. Additionally, if your symptoms persist or worsen, it would be wise to consult your healthcare provider for further evaluation.

In summary, while it is theoretically possible to become pregnant while on birth control pills, your consistent use and the absence of factors that typically reduce their effectiveness suggest that your risk is low. However, if you have any doubts or experience unusual symptoms, seeking medical advice is always a good practice.
