Enlarged testicles?
Hello Doctor, I have recently noticed that my testicles sometimes feel swollen and painful.
Riding a bike or walking feels uncomfortable, and when I measure them myself, they seem to have increased in size.
Previously, they measured about 3.5 cm, but recently they are close to 5 cm.
However, the enlargement is not unilateral; both sides appear to be the same.
What could be the cause of this? Is there a possibility that it could be testicular cancer? I have read online that most cases of testicular cancer involve unilateral swelling, while mine is bilateral, and I haven't felt any lumps.

Hello: Testicular cancer is more common on one side, but if the testicle enlarges within 1-2 months, it is necessary to seek medical attention from a urologist.
Wishing you good health.

More Info
Testicular enlargement can be a concerning symptom, and it’s important to approach this issue with a thorough understanding of potential causes and implications. Based on your description, you are experiencing bilateral testicular enlargement accompanied by discomfort, which is indeed a valid concern that warrants further investigation.
First, it’s essential to note that testicular size can vary among individuals, and slight changes in size may not always indicate a serious condition. However, significant changes, especially when accompanied by pain or discomfort, should be evaluated by a healthcare professional.
Possible Causes of Testicular Enlargement
1. Epididymitis: This is an inflammation of the epididymis, often caused by infections, including sexually transmitted infections (STIs). Symptoms can include swelling, pain, and sometimes fever. This condition can lead to discomfort and changes in testicular size.
2. Orchitis: Similar to epididymitis, orchitis is an inflammation of the testicle itself, which can also be due to viral or bacterial infections. Mumps virus is a common cause of viral orchitis.
3. Hydrocele: A hydrocele is a fluid-filled sac surrounding a testicle that can cause swelling. It is usually painless and can occur on one or both sides. Hydroceles can develop due to injury or inflammation.
4. Varicocele: This condition involves enlarged veins within the scrotum, similar to varicose veins. It can cause a feeling of heaviness or discomfort and may lead to changes in size.
5. Testicular Tumors: While you mentioned that you have not felt any lumps, it is still important to consider that testicular cancer can present with enlargement. Typically, testicular cancer may present as a painless lump, but it can also cause swelling or changes in size without a distinct mass.
6. Testicular torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. It usually presents with sudden severe pain and swelling, but it is less likely in your case given the bilateral nature of your symptoms.
Concerns Regarding Testicular Cancer
You are correct that testicular cancer often presents as unilateral enlargement. However, it is not impossible for bilateral conditions to occur, especially in the early stages. The absence of a palpable mass does not rule out the possibility of cancer, as some tumors can be small or located in areas that are difficult to examine.
Recommendations
Given your symptoms, it is crucial to seek medical attention promptly. A healthcare provider will likely perform a physical examination and may recommend imaging studies, such as an ultrasound, to assess the structure of the testicles and surrounding tissues. Blood tests, including tumor markers, may also be conducted to rule out malignancy.
Conclusion
In summary, while there are several benign conditions that could explain your symptoms, it is essential to rule out serious conditions, including testicular cancer. Early diagnosis and treatment are key to managing any potential issues effectively. Please make an appointment with a urologist or your primary care physician to discuss your symptoms in detail and undergo the necessary evaluations. Your health and peace of mind are paramount, and addressing these concerns with a professional is the best course of action.
