Phimosis issue
Hello, doctor.
I am in a state of phimosis regardless of whether my foreskin is retracted or not.
However, my penis tends to retract into my body when the foreskin is not retracted.
Could you please explain why this happens and how I should treat it? Additionally, I have previously tried to push the foreskin back, but it feels tight and painful.
However, when I slowly push it back under normal conditions and expose the glans, I notice that my penis retracts and even becomes engorged.
Why does this occur? Also, when I masturbate with the foreskin retracted, I experience premature ejaculation.
What should I do about this?

Dr. Sun Hongshun reply Urology
You have an issue with phimosis, and there may also be a slight excess of lower limb adiposity, which could be causing you discomfort.
It is recommended that you consult a hospital regarding circumcision surgery.

More Info
Phimosis is a condition characterized by the inability to retract the foreskin over the glans (the head) of the penis. This condition can be present from birth or develop over time due to scarring or inflammation. Understanding the causes, symptoms, and treatment options for phimosis is crucial for managing this condition effectively.
Causes of Phimosis
1. Congenital Phimosis: This is when the foreskin is naturally tight and cannot be retracted at birth. In many cases, this condition resolves itself as the child grows, with the foreskin becoming more pliable.
2. Acquired Phimosis: This can occur due to scarring from infections, inflammation (such as balanitis), or trauma. Conditions like diabetes can increase the risk of infections that may lead to phimosis.
3. Poor Hygiene: Inadequate cleaning under the foreskin can lead to the buildup of smegma, which can cause irritation and inflammation, contributing to phimosis.
Symptoms of Phimosis
- Inability to Retract the Foreskin: This is the primary symptom. If you find it painful or impossible to pull back the foreskin, this indicates phimosis.
- Pain or Discomfort: You may experience pain during erections or sexual activity, as you mentioned with early ejaculation.
- Swelling or Redness: In cases of inflammation or infection, there may be visible signs such as swelling or redness of the foreskin or glans.
- Recurrent Infections: Frequent urinary tract infections or balanitis can also be a symptom of phimosis.
Treatment Options
1. Topical Steroid Creams: These can help to soften the foreskin and make it easier to retract. Your doctor may prescribe a steroid cream to apply to the foreskin for a few weeks.
2. Gentle Stretching Exercises: Gradual stretching of the foreskin can sometimes help. However, this should be done carefully to avoid pain or injury. It’s essential to do this under the guidance of a healthcare professional.
3. Circumcision: In cases where phimosis is severe or does not respond to other treatments, circumcision (the surgical removal of the foreskin) may be recommended. This is a definitive solution to the problem.
4. Preputioplasty: This is a less invasive surgical option that involves widening the opening of the foreskin without removing it. This can be an option for those who wish to retain their foreskin.
Addressing Your Concerns
From your description, it seems you are experiencing significant discomfort and complications related to phimosis. The fact that your penis retracts into your body when not erect and that you feel tightness and pain when attempting to retract the foreskin suggests that you may benefit from medical intervention.
The early ejaculation you mentioned could be related to anxiety or discomfort during sexual activity, which is not uncommon in individuals with phimosis. It’s essential to address both the physical and psychological aspects of your condition.
Recommendations
1. Consult a Urologist: It’s crucial to see a specialist who can evaluate your condition thoroughly. They can provide a tailored treatment plan based on the severity of your phimosis and any associated symptoms.
2. Avoid Forcing Retraction: Do not attempt to forcefully retract the foreskin, as this can lead to pain, tearing, or further complications.
3. Practice Good Hygiene: Ensure you are cleaning the area gently to prevent infections, which can exacerbate phimosis.
4. Discuss Sexual Health: If early ejaculation is a concern, discussing this with your healthcare provider can lead to strategies or treatments that can help.
In conclusion, phimosis can be a manageable condition with the right approach. Seeking professional medical advice is the best step forward to ensure your health and comfort.
