Retinal degeneration

Hello Doctor: I have high myopia with a left eye prescription of -1075 degrees (astigmatism -150) and a right eye prescription of -1100 degrees (astigmatism -175).
I also have dry eye syndrome and floaters, as well as retinal degeneration (there is a retinal tear in my left eye that was treated with laser reinforcement on June 29, and the right eye has lattice degeneration which was previously treated with laser and is currently stable).
I would like to ask what problems retinal degeneration can cause for the eyes.
Recently, I have been experiencing difficulty seeing clearly in low light or darkness at night; could this be considered night blindness? How can I address this issue? I read online that severe night blindness can lead to insomnia, and I am quite worried about this.
Additionally, over the past two days, when I close my eyes while sleeping, I feel like there is a flickering light in my left eye (the one that underwent laser reinforcement).
Should I have this checked by a doctor? (I had an examination on July 13, and the doctor said there were no issues, but I am scheduled for another retinal check in August.) I apologize for the trouble, and thank you for your response!

Kevin, 20~29 year old female. Ask Date: 2018/07/22

Dr. He Jianxian reply Ophthalmology

Night blindness, aside from being caused by nutritional deficiencies (such as vitamin A deficiency), is mostly related to genetics or hereditary factors, which is why it often manifests from a young age and is less common in older individuals.
For your reference, if you experience any abnormal visual disturbances such as flashes or floaters, it is advisable to undergo a dilated eye examination for safety.

Hello! Thank you for reaching out with your concerns regarding retinal degeneration and its associated symptoms. Given your history of high myopia, dry eye syndrome, and the presence of retinal issues, it’s understandable that you have questions about the implications of retinal degeneration and how it may affect your vision.

Understanding Retinal Degeneration
Retinal degeneration refers to the gradual deterioration of the retina, which is the light-sensitive layer at the back of the eye. This condition can lead to various visual impairments, including blurred vision, difficulty seeing in low light, and potential vision loss. In your case, the presence of retinal holes and degeneration can pose risks, especially if they progress or lead to complications such as retinal detachment.

Symptoms and Risks
The symptoms you are experiencing, particularly difficulty seeing in low light or darkness, can indeed be related to night blindness (nyctalopia). Night blindness is often associated with retinal diseases and can occur when the rods in the retina, which are responsible for vision in low light, are affected. Given your retinal degeneration, it is plausible that this could be contributing to your symptoms.
The flashing lights you describe in your left eye, especially after undergoing laser treatment, could be a sign of ongoing retinal changes or irritation. This is something that warrants further investigation, as it could indicate that the retina is still responding to the previous treatment or that there may be new developments that need to be assessed.

Solutions and Recommendations
1. Regular Monitoring: It’s crucial to maintain regular follow-ups with your ophthalmologist. Since you have a history of retinal issues, your doctor may recommend more frequent examinations to monitor any changes in your retina.

2. Night Vision Aids: For your difficulty seeing in low light, consider using brighter lighting when possible and avoid situations where you might need to see in the dark. There are also specialized glasses that can enhance contrast and improve night vision.

3. Addressing Flashes of Light: The flashes of light you are experiencing should be discussed with your ophthalmologist as soon as possible. They may want to perform a thorough examination, including a dilated fundus exam, to assess the retina's condition and rule out any serious complications.

4. Managing Dry Eye: Since you also have dry eye syndrome, managing this condition can help improve your overall comfort and visual clarity. Use lubricating eye drops as recommended by your doctor, and consider lifestyle changes that can alleviate dry eye symptoms, such as taking breaks from screens and ensuring adequate hydration.

5. Lifestyle Adjustments: Maintaining a healthy lifestyle, including a balanced diet rich in antioxidants (like vitamins A, C, and E), omega-3 fatty acids, and regular exercise, can support overall eye health. Additionally, protecting your eyes from UV light by wearing sunglasses outdoors can help reduce further retinal damage.

Conclusion
While retinal degeneration can pose significant risks to your vision, proactive management and regular check-ups can help mitigate these risks. It’s essential to communicate openly with your ophthalmologist about any new symptoms or concerns, especially regarding the flashes of light and your night vision difficulties. They can provide tailored advice and treatment options based on your specific situation. Remember, early detection and intervention are key in managing eye health effectively.
