Please consult a doctor?
Hello doctor, I am currently 7 months pregnant and have been experiencing shoulder and back pain in the area around my right scapula for the past few months.
It hurts when I touch or press on it, and sometimes the left side of my back also hurts.
I am concerned whether this could be an early sign of lung cancer.
Is it safe for me to get imaging done? I have a history of smoking but have just recently quit.
Thank you.
Xiao Mei, 40~49 year old female. Ask Date: 2018/07/27
Dr. Jian Shuntian reply Pulmonology
Hello: Based on the description, it seems more like muscle pain or myofascial pain.
If you are still concerned, a chest X-ray should be performed with fetal protection measures in place.
Thank you.
Sincerely, Jian Shun Tian.

Shoulder pain during pregnancy is a common complaint and can arise from various factors, including hormonal changes, weight gain, and postural adjustments as the body accommodates the growing fetus. It is understandable to be concerned about any pain during pregnancy, especially with a history of smoking, which can contribute to anxiety about potential health issues like lung cancer. However, it is essential to approach this situation with a clear understanding of the typical causes of shoulder pain during pregnancy and the appropriate steps to take.
Firstly, the pain you are experiencing around the shoulder blade area is likely related to the physical changes your body is undergoing. As your pregnancy progresses, the additional weight can strain your muscles and joints, particularly in the back and shoulders. The hormonal changes that occur during pregnancy can also lead to increased laxity in the ligaments, which may contribute to discomfort. Furthermore, as your center of gravity shifts, it can lead to poor posture, which often exacerbates shoulder and back pain.
In your case, the pain localized to the right shoulder blade and sometimes the left side could be due to muscle strain or tension. It is not uncommon for pregnant individuals to experience muscle tightness or spasms in the upper back and shoulders, especially if they are less active or have altered their usual physical activities due to pregnancy. Stress and anxiety can also manifest physically, leading to muscle tension in these areas.
Regarding your concern about lung cancer, while it is crucial to be vigilant about health, shoulder pain alone is not typically a symptom of lung cancer. Common symptoms of lung cancer may include persistent cough, unexplained weight loss, chest pain, and difficulty breathing. If you are only experiencing shoulder pain without these additional symptoms, it is less likely to be related to lung cancer. However, your history of smoking does warrant regular check-ups and discussions with your healthcare provider about lung health.
As for imaging studies, such as X-rays or other forms of radiation, it is generally advised to avoid them during pregnancy unless absolutely necessary. The potential risks to the developing fetus must be weighed against the benefits of obtaining the imaging. If your healthcare provider believes that imaging is necessary, they will take precautions to minimize any exposure.
To manage your shoulder pain, consider the following approaches:
1. Physical Therapy: Engaging in physical therapy can help strengthen the muscles around your shoulders and back, improve posture, and provide relief from pain.
2. Gentle Exercise: Activities such as prenatal yoga or swimming can help alleviate tension and improve flexibility without putting undue stress on your body.
3. Heat Therapy: Applying a warm compress to the affected area can help relax tight muscles and relieve pain.
4. Massage: Prenatal massage from a qualified therapist can provide relief and help reduce muscle tension.
5. Posture Awareness: Being mindful of your posture, especially when sitting or standing for extended periods, can help prevent further strain on your shoulders.
6. Stress Management: Techniques such as deep breathing, meditation, or mindfulness can help reduce overall stress, which may contribute to muscle tension.
If your shoulder pain persists or worsens, or if you develop new symptoms, it is essential to consult your healthcare provider for a thorough evaluation. They can help determine the underlying cause of your pain and recommend appropriate treatment options tailored to your situation. Remember, while it is natural to worry about your health during pregnancy, many common discomforts can be managed effectively with the right approach.
