Postoperative complications of grade II varicocele surgery?
Hello Dr.
Du: Fourteen years ago, I underwent surgery for left-sided varicocele (traditional surgery, with the incision at the upper corner of the pubic hair).
Last year, during an examination at Chang Gung Memorial Hospital, it was found that the varicocele had recurred (again on the left side), so I underwent a second surgery (microsurgery, with the incision located lower, near the side of the penis).
Six months after the surgery, I began to experience pain and discomfort in the area between the new and old incisions.
Initially, the pain was occasional, but it has become more frequent and has now progressed to daily pain.
At its worst, it feels like the blood vessels are about to rupture, and sometimes the pain radiates up into the deep lower abdomen.
Another characteristic is that the pain intensifies when I consume anticoagulant medications, foods, or alcohol.
I returned to the doctors at Chang Gung Hospital for an examination, but they said there was no way to assess the area of my pain.
The only examination they could perform was a scrotal ultrasound, which showed no abnormalities and no varicocele.
They could only prescribe me painkillers, but I have not seen any improvement.
The continuous pain has lasted for over seven months and seems to be worsening.
I am only able to manage it with painkillers, and I am really worried.
Could this be due to internal bleeding? What could be causing this? Are there any doctors in Taipei I can consult for help? Thank you.
Da Bao, 30~39 year old female. Ask Date: 2018/08/05
Dr. Du Shixi reply Urology
Hello: Post-operative wound pain is not uncommon and is often caused by nerve and vascular influences.
Since some time has passed, the likelihood of internal bleeding is low.
In addition to medication, it is important to pay attention to lifestyle; avoid sitting or standing for prolonged periods, and make sure to get up and move around after sitting for a while.
Avoid excessive straining of the abdomen, and maintain a light diet.
It may be beneficial to increase the intake of foods rich in vitamin B.
Strenuous exercise should be avoided to see if there is any improvement.
Are there any doctors in Taipei that I can seek help from? Unfortunately, this platform does not recommend specific physicians.
Wishing you good health.

Post-surgery complications, especially after varicocele surgery, can be quite distressing and may significantly impact a patient's quality of life. In your case, it appears that you are experiencing persistent pain in the area between your old and new surgical sites, which has progressively worsened over the past several months. This situation warrants a thorough understanding of potential causes and appropriate steps to seek further medical assistance.
Firstly, it is important to recognize that pain following varicocele surgery can arise from several factors. One common issue is the formation of scar tissue, which can lead to nerve entrapment or irritation. This phenomenon is often referred to as "neuropathic pain," where the nerves in the area become hypersensitive due to the surgical trauma. Additionally, the surgical site may develop adhesions, which are bands of scar tissue that can bind organs and tissues together, potentially causing discomfort and pain.
Another possibility is that the pain you are experiencing could be related to the underlying vascular structures. Even after surgery, there may be residual or new vascular issues that could lead to discomfort. The fact that your pain intensifies with anticoagulant medications or alcohol consumption suggests that there may be a vascular component to your symptoms. These substances can affect blood flow and pressure in the area, potentially exacerbating any underlying issues.
Given that you have undergone two surgeries and are still experiencing significant pain, it is crucial to pursue further evaluation. While the ultrasound did not reveal any abnormalities, it may be beneficial to consider additional imaging studies, such as a Doppler ultrasound or MRI, to assess the blood vessels and surrounding tissues more comprehensively. These tests can help identify any vascular complications, such as thrombosis or other issues that may not have been visible on the initial ultrasound.
It is also essential to communicate openly with your healthcare provider about the severity and nature of your pain. If your current physician is unable to provide satisfactory answers or solutions, seeking a second opinion from a specialist in urology or a pain management expert may be advisable. In Taipei, there are several reputable hospitals and clinics where you can find specialists experienced in treating post-surgical complications related to varicocele. Institutions like Taipei Medical University Hospital or National Taiwan University Hospital have urology departments that may offer the expertise you need.
In the meantime, managing your pain effectively is crucial. While you mentioned that over-the-counter pain medications have not provided relief, discussing alternative pain management strategies with your doctor could be beneficial. Options may include physical therapy, nerve blocks, or other interventional pain management techniques that could help alleviate your symptoms.
Lastly, it is vital to monitor any new symptoms that may arise, such as swelling, changes in color, or increased pain, as these could indicate complications such as infection or internal bleeding. If you experience any of these symptoms, seek immediate medical attention.
In summary, persistent pain following varicocele surgery can stem from various causes, including scar tissue formation, nerve irritation, or vascular issues. It is essential to pursue further evaluation and consider seeking a second opinion if your current treatment plan is not effective. By taking these steps, you can work towards finding relief and improving your overall well-being.
