Experiencing chest tightness upon waking?


Hello, Doctor He: I would like to ask why I often experience chest tightness upon waking up.
Sometimes it occurs, and sometimes it does not, with varying degrees of severity.
Occasionally, I wake up in the middle of the night feeling a heavy pressure on my chest, as if a stone is resting on it.
This issue has been troubling me for quite some time.
I visited a cardiologist two years ago and again last year.
Two years ago, the doctor performed a chest X-ray and a pulmonary function test, and after the examinations, the doctor concluded that there were no abnormalities and suggested it might be related to stress.
Last year, the doctor conducted an electrocardiogram (ECG) and also said it appeared normal, again suggesting it could be a stress-related issue.
I usually experience chest tightness in the morning, which significantly eases by the afternoon.
I heard from nursing staff that if an ECG is done in the afternoon, it might not show any issues, which is why some people go for testing immediately after experiencing symptoms in the morning.
However, since my symptoms are intermittent, I did not pursue further ECG testing.
The doctor also mentioned that it could be gastroesophageal reflux disease (GERD).
Due to work commitments, I often had dinner around 8 or 9 PM two years ago, but since changing jobs, my schedule has become more regular.
Still, I continue to experience this issue, so it may not be caused by undigested food leading to GERD.
Additionally, I do not feel any typical GERD symptoms such as heartburn or sour belching.
The most I experience is some phlegm in my throat after napping on my stomach post-lunch.
Based on this, do you think my symptoms could be related to stress? I generally do not have significant stress at work, but could it still be due to some underlying, intangible stress? Thank you.
