Itchy and painful anus with discharge?

Due to being abroad and unable to return home for medical treatment, I initially experienced itching around the anal area, which has now progressed to both itching and pain.
There are red sores near the anus, with some small white spots resembling blisters, and there is discharge with an unpleasant odor.
Recently, I have even felt sharp pain in the genital area while urinating.
What could be wrong with me, and what should I do?

Ting, 20~29 year old female. Ask Date: 2018/08/24

Dr. Li Yufen reply Dermatology

Hello: It is not possible to accurately diagnose a skin condition based solely on a textual description.
If there is pain during urination, it may be necessary to rule out a urinary tract infection.
If the situation does not improve or continues to worsen, it may be advisable to seek medical diagnosis and treatment abroad.
Thank you!

More Info

Anal itching, pain, and unpleasant discharge can be distressing symptoms that may indicate a variety of underlying conditions. Given your description of symptoms—itching, pain, red sores, small white spots resembling blisters, and foul-smelling discharge—it's essential to consider several potential causes and appropriate actions to take.

Possible Causes
1. Hemorrhoids: Swollen veins in the rectal area can cause itching, pain, and bleeding. They may also lead to discomfort during urination if they are inflamed.

2. Anal Fissures: Small tears in the lining of the anus can result from straining during bowel movements, leading to pain and bleeding. These fissures can also become infected, causing additional symptoms.

3. Infections: Bacterial, viral, or fungal infections can lead to symptoms like itching, pain, and discharge. Conditions such as herpes simplex virus (which can cause blisters) or sexually transmitted infections (STIs) like gonorrhea or chlamydia could be possibilities.

4. Skin Conditions: Conditions such as eczema or psoriasis can affect the anal area, leading to itching and discomfort.

5. Pinworms: Particularly in children, pinworm infections can cause significant anal itching, especially at night.

6. Yeast Infections: A yeast infection can cause itching and discharge, often accompanied by a foul odor.

Recommended Actions
1. Maintain Hygiene: Keep the anal area clean and dry. Use mild soap and water, and avoid harsh soaps or scented products that may irritate the skin further.

2. Avoid Irritants: Stay away from perfumed products, harsh toilet paper, and tight clothing that can exacerbate irritation.

3. Over-the-Counter Treatments: You may consider using over-the-counter hydrocortisone cream to reduce inflammation and itching. Antifungal creams can be helpful if a yeast infection is suspected.

4. Warm Sitz Baths: Taking warm baths can soothe the area and help relieve pain and itching.
5. Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or acetaminophen can help manage pain.

6. Dietary Adjustments: Ensure you are consuming a high-fiber diet to prevent constipation and straining during bowel movements, which can worsen symptoms.

7. Avoid Scratching: Although it may be tempting, scratching can worsen irritation and lead to infection.

When to Seek Medical Attention
Given the severity of your symptoms, especially the presence of red sores and discharge with an unpleasant odor, it is crucial to seek medical attention as soon as possible. If you are unable to return to your home country for treatment, consider visiting a local healthcare provider or clinic. They can perform a physical examination, possibly including a rectal exam, to determine the underlying cause of your symptoms and recommend appropriate treatment.

Conclusion
While some conditions may resolve with home care, others may require prescription medications or further intervention. It is essential to address these symptoms promptly to prevent complications and ensure your comfort. If you experience worsening symptoms, such as increased pain, fever, or significant bleeding, seek immediate medical care. Your health is paramount, and timely intervention can lead to better outcomes.
