Postoperative issues: lipoma and subcutaneous cyst?

Hello, Doctor: A few weeks ago, I had a sebaceous cyst excised from my face, leaving a wound approximately 1 to 2 centimeters.
It has been nearly a month since the stitches were removed, but there is still a slight indentation.
I have been rinsing it daily with saline solution.
Recently, I noticed some serous fluid secretion, but in the past few days, it feels a bit painful, similar to a pimple, and I have expressed a small amount of pus.
What should I do, and what should I be aware of?

Xiao Yu, 10~19 year old female. Ask Date: 2018/09/14

After surgery, particularly for cyst removal, it's common to experience some concerns regarding wound healing and skin issues. Based on your description, it sounds like you are dealing with a few important aspects of post-operative care that need to be addressed.

Firstly, the presence of a small opening or "little hole" at the surgical site is not uncommon, especially in the early stages of healing. Surgical wounds can take time to fully close, and the healing process can vary significantly from person to person. The fact that you have been rinsing the area with saline solution is a good practice, as it helps to keep the wound clean and can promote healing.

However, the development of a painful area that resembles a pimple and the presence of pus are concerning signs. This could indicate a localized infection at the site of the surgery. Infections can occur if bacteria enter the wound, particularly if the area is not kept clean or if there is any disruption to the healing tissue. The pus you mentioned is a sign that your body is responding to this potential infection, and it is crucial to address this promptly.

Here are some steps you should consider taking:
1. Consult Your Surgeon or Healthcare Provider: Given the symptoms you are experiencing, it is advisable to return to your healthcare provider for an evaluation. They may need to assess the wound for signs of infection and determine if any further treatment is necessary. This could include draining the area if there is an abscess or prescribing antibiotics if an infection is confirmed.

2. Keep the Area Clean: Continue to clean the wound gently with saline or a mild antiseptic solution as advised by your healthcare provider. Avoid using harsh chemicals or scrubbing the area, as this can irritate the skin and delay healing.

3. Avoid Picking or Squeezing: It may be tempting to squeeze the area that resembles a pimple, but this can exacerbate the problem by pushing bacteria deeper into the tissue and potentially leading to more significant infection or scarring.

4. Monitor for Other Symptoms: Keep an eye on the wound for any signs of worsening infection, such as increased redness, swelling, warmth, or fever. If you notice any of these symptoms, seek medical attention immediately.

5. Follow Post-Operative Instructions: Ensure you are following any specific post-operative care instructions provided by your surgeon. This may include restrictions on physical activity, dietary recommendations, or specific wound care protocols.

6. Consider Scar Management: Once the wound has healed, you may want to discuss scar management options with your healthcare provider. This can include silicone gel sheets, pressure therapy, or other treatments that can help minimize the appearance of scars.

In summary, while some post-operative issues are normal, the symptoms you are experiencing warrant further evaluation by a healthcare professional. Prompt attention to potential infections can prevent complications and promote better healing outcomes. Always prioritize communication with your healthcare provider regarding any concerns you have during your recovery process.
