Premature ejaculation
Hello, doctor.
I have been experiencing some issues with my current girlfriend (my first partner) during sexual intercourse (about three or four times on different nights).
1.
On one or two occasions during foreplay, I was unable to achieve an erection.
2.
Aside from the aforementioned situations, I have been able to achieve an erection, but when the glans comes into contact with her body after foreplay, I ejaculate (without penetration).
What should I do, doctor? I have visited a urology clinic, and the doctor mentioned that it might be a psychological issue (I also had blood tests for testosterone levels, which were normal).

Dr. Chen Jiansheng reply Urology
After taking a break, try again.
If it still doesn't work, you may consider pharmacological treatment.

More Info
Premature ejaculation (PE) is a common sexual dysfunction that can affect men of all ages. It is characterized by the inability to control ejaculation during sexual activity, often resulting in ejaculation occurring sooner than desired. Understanding the causes and potential solutions for premature ejaculation is essential for addressing this issue effectively.
Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, stress, and relationship issues can significantly impact sexual performance. The pressure to perform or fear of not satisfying a partner can lead to premature ejaculation. In your case, since you mentioned that you are with your first girlfriend, the anxiety of being with someone new could be contributing to your experience.
2. Physiological Factors: Hormonal imbalances, particularly involving testosterone, can play a role in sexual function. However, since your testosterone levels were reported as normal, this may not be a contributing factor for you. Other physiological issues, such as hypersensitivity of the penis or certain neurological conditions, can also lead to PE.
3. Medical Conditions: Certain medical conditions, including prostatitis or thyroid issues, can affect ejaculation. It’s important to rule out any underlying medical problems through a thorough examination by a healthcare professional.
4. Lifestyle Factors: Substance use, including alcohol and recreational drugs, can impact sexual performance. Additionally, lack of physical fitness and poor diet may contribute to sexual dysfunction.
Solutions and Management Strategies
1. Behavioral Techniques:
- Start-Stop Technique: This involves stimulating the penis until you feel close to ejaculation, then stopping until the urge decreases. This can help you gain better control over your ejaculation.
- Squeeze Technique: Similar to the start-stop method, this involves squeezing the base of the penis to reduce the urge to ejaculate.
2. Psychological Counseling: Since psychological factors are often at play, speaking with a therapist or counselor who specializes in sexual health can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and improve sexual confidence.
3. Medication: There are medications available that can help delay ejaculation. Selective serotonin reuptake inhibitors (SSRIs) are commonly prescribed for this purpose. Topical anesthetics can also be applied to the penis to reduce sensitivity.
4. Communication with Partner: Open communication with your partner about your experiences and feelings can help reduce anxiety and improve intimacy. Understanding and support from your partner can alleviate some of the pressure you may feel.
5. Lifestyle Changes: Engaging in regular physical activity, maintaining a healthy diet, and reducing alcohol and drug use can improve overall sexual health and performance.
6. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can enhance control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles that control urination.
Conclusion
Premature ejaculation is a manageable condition, and many men experience it at some point in their lives. It’s important to approach this issue with patience and an open mind. Since you have already consulted a urologist and ruled out significant medical concerns, focusing on psychological factors and behavioral techniques may be the next best step. If symptoms persist, consider seeking further evaluation or treatment options. Remember, sexual health is an integral part of overall well-being, and addressing these concerns can lead to improved sexual experiences and relationships.
