Drug side effects

Hello Dr.
Hong,
The ophthalmologist prescribed Aifermu P sterile ophthalmic solution 0.15, to be used twice daily.
I would like to ask:
1) This medication is an adrenal stimulant.
What long-term effects might it have on my adrenal glands?
2) Why is an eye medication related to the adrenal glands and can it affect the entire body?
3) My pulse is somewhat slow, below 60, sometimes dropping to 50.
I have heard that this medication can cause a slower heart rate; is that true?
4) After using the medication, I feel more fatigued.
If I switch to once daily use, although the efficacy may be reduced, can it still be effective in principle? I suspect that my eye pressure is not high, and insufficient blood flow to the eyeball may also be a significant reason for optic nerve damage.

Dr.
Hong, I have carefully read the medication instructions, but I still do not understand the above questions.
I look forward to your response to clarify my doubts.
Thank you.

Sincerely,
Guding, 9/27

Dr. Hong Kunyi reply Pharmacology

Hello Mr.
Guding: This medication is Alphagan P (Brimonidine Tartrate Ophthalmic Solution 0.15% 5ml), indicated for open-angle glaucoma or ocular hypertension.
You may have some concerns regarding its mechanism of action, but when used under the supervision of an ophthalmologist, it should not cause any adverse effects, so please use it with confidence.
The side effects of this medication may include allergic conjunctivitis, conjunctival hyperemia, and eye itching, which are generally mild.
If you experience any discomfort, please remember to inform your ophthalmologist.
This medication belongs to the class of alpha agonists, which may reduce heart rate and lower blood pressure.
Caution is advised when used in conjunction with certain topical and systemic beta-blockers, antihypertensives, and cardiac medications.
If you have any existing conditions during your outpatient visit, please proactively inform your physician.
When used alone, this eye drop may lower your heart rate and blood pressure, so you may want to discuss with your ophthalmologist whether to continue using it or switch to another type of eye drop.
I hope this answers your questions, and I wish you good health and well-being.

More Info

Hello Mr. Gu,
Thank you for your detailed questions regarding the use of Alphagan P (brimonidine tartrate) eye drops. I understand your concerns about the potential side effects, especially regarding adrenal health and heart rate. Let’s address your questions one by one.

1. Impact on Adrenal Health: Alphagan P is an alpha-2 adrenergic agonist, which means it stimulates certain receptors in the body that can lead to a decrease in the production of aqueous humor in the eye, thereby lowering intraocular pressure. While it is true that systemic absorption of this medication can occur, the concentration in the bloodstream is generally low when used as directed. Long-term use of this medication is not typically associated with significant adverse effects on adrenal function. However, if you have pre-existing adrenal issues or are on other medications that affect adrenal function, it’s essential to discuss this with your healthcare provider.

2. Connection Between Eye Drops and Adrenal Function: The reason eye drops like Alphagan P can have systemic effects is due to their absorption into the bloodstream. When applied to the eye, some of the medication can be absorbed through the conjunctiva and cornea, leading to systemic circulation. This is why medications that act on the adrenergic system can influence heart rate and blood pressure. The alpha-2 agonist action can lead to a decrease in sympathetic outflow, which can affect various body systems, including the cardiovascular system.

3. Effect on Heart Rate: You mentioned that your heart rate is sometimes below 60 beats per minute, occasionally dropping to 50. Alphagan P can indeed cause bradycardia (a slower than normal heart rate) as a side effect. This is particularly relevant if you are taking other medications that can also lower heart rate or if you have underlying heart conditions. It’s crucial to monitor your heart rate and discuss any significant changes with your healthcare provider, especially if you experience symptoms like dizziness or fatigue.

4. Adjusting Dosage: If you are experiencing fatigue or other side effects, discussing a dosage adjustment with your doctor is a wise decision. Reducing the frequency of application from twice a day to once a day may still provide some therapeutic benefit, although it might not be as effective in controlling intraocular pressure. Your observation about your eye pressure being normal is important; if your eye pressure is well-controlled, your doctor may consider a less frequent dosing schedule. However, it’s essential to have this conversation with your eye care provider to ensure that any changes in your regimen are safe and appropriate for your specific situation.

In summary, while Alphagan P can have systemic effects, its impact on adrenal health and heart rate is generally minimal when used as prescribed. However, individual responses can vary, and it’s essential to maintain open communication with your healthcare providers about any concerns or side effects you experience. Regular monitoring of your heart rate and discussing any significant changes with your doctor will help ensure your overall health and safety while using this medication.

Best wishes for your health, and I hope this information helps clarify your concerns.
