Oral 150 mg fluconazole?
Hello, doctor.
After a consultation, I was diagnosed with vulvovaginal candidiasis.
After completing the treatment with suppositories, my discharge was clear and normal.
I then continued maintenance treatment with oral fluconazole 150 mg once a week.
However, I noticed that the day after taking the medication, my discharge appears slightly yellow.
There is a small amount of yellow on the toilet paper, and when I separate it with my fingers, it turns white (I am currently close to ovulation).
The yellow color is noticeably different from what I have seen before, and it is a small amount that does not stain my underwear; I only notice it when I wipe the vaginal area with toilet paper.
Is this normal? Can taking oral fluconazole cause a small amount of yellow discharge? Could the yellow color be due to the expulsion of dead fungi? Thank you for taking the time to answer my questions.

Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, oral fluconazole generally does not cause secretions that are slightly yellow; it may indicate inflammation.
If you continue to feel unwell, it is advisable to consult a gynecologist.
Wishing you good health.

The presence of yellow discharge after taking oral fluconazole for a vaginal yeast infection can be concerning, but it is important to understand the context and potential causes of this symptom. Fluconazole is an antifungal medication commonly used to treat yeast infections, and it works by inhibiting the growth of fungi, specifically Candida species, which are responsible for most yeast infections.
After completing a treatment regimen with fluconazole, it is not uncommon for patients to notice changes in their vaginal discharge. The discharge may vary in color, consistency, and volume as the body responds to the treatment and the yeast infection resolves. In your case, the yellow discharge you are experiencing could be attributed to several factors:
1. Normal Variation: Vaginal discharge can naturally vary in color and consistency throughout the menstrual cycle due to hormonal changes. During the ovulation phase, which you mentioned is approaching, it is common for women to experience changes in discharge, including a more opaque or slightly colored discharge.
2. Yeast Die-Off: As the fluconazole works to eliminate the yeast infection, the dying yeast cells may be expelled from the body, potentially leading to a change in discharge color. This could explain the presence of yellow discharge, as it may contain remnants of the yeast.
3. Other Infections: While fluconazole is effective against yeast infections, it does not protect against other types of infections, such as bacterial vaginosis or sexually transmitted infections (STIs). If the yellow discharge persists or is accompanied by other symptoms such as itching, burning, or a strong odor, it may be indicative of another infection that requires medical attention.
4. Hormonal Influence: Hormonal fluctuations during your menstrual cycle can also affect the characteristics of vaginal discharge. Increased estrogen levels around ovulation can lead to more abundant and possibly different-colored discharge.
5. Irritation or Allergic Reaction: Sometimes, the use of medications or even personal hygiene products can cause irritation or an allergic reaction, leading to changes in discharge. If you have recently changed any products or routines, this could be a contributing factor.
In conclusion, while the yellow discharge you are experiencing after taking oral fluconazole may be normal and could be related to the treatment or hormonal changes, it is essential to monitor the situation closely. If the discharge continues, increases in volume, or is accompanied by other symptoms such as discomfort, odor, or itching, it is advisable to consult with a healthcare provider. They may recommend further evaluation, including a pelvic examination or laboratory tests, to rule out any other underlying conditions.
Overall, maintaining open communication with your healthcare provider about any changes in your symptoms is crucial for ensuring your health and well-being.
