Itching of the nipples and areola?
Hello, doctor! My 15-year-old daughter has been experiencing severe itching in her nipple recently.
Upon inspection, I noticed that the right nipple has turned dark, and there is a small, soft, sticky, and moist bump near the nipple (within the areola area) that secretes fluid.
What could be the issue? The left side is completely normal and shows no signs of discoloration.

Dr. Yuan Tianmin reply Breast and Thyroid
That is a normal distribution of glands on the areola and does not require concern.

Itchy nipples, particularly in teenagers, can be concerning and may arise from various causes. In your daughter's case, the symptoms you describe—itchiness, discoloration (darkening), and the presence of a small, soft, moist bump that secretes fluid—could indicate a few potential conditions.
1. Hormonal Changes: At the age of 15, your daughter is likely experiencing significant hormonal changes due to puberty. These changes can lead to various skin conditions, including increased sensitivity and irritation of the nipple area. Hormonal fluctuations can also cause changes in pigmentation, which might explain the darkening of the right nipple.
2. Infection: The presence of a soft, moist bump that secretes fluid raises the possibility of an infection, such as a localized skin infection or an abscess. Conditions like folliculitis (inflammation of hair follicles) or a sebaceous cyst could also present with similar symptoms. If the bump is red, swollen, or painful, it would be advisable to seek medical attention.
3. Dermatitis: Contact dermatitis, which is an allergic reaction to substances that come into contact with the skin, could also be a cause of itchiness. This could be due to soaps, lotions, or even fabrics that irritate the skin. If your daughter has recently changed any personal care products, this could be a contributing factor.
4. Paget's Disease of the Nipple: Although rare, Paget's disease can cause changes in the nipple and surrounding area, including itching, discoloration, and discharge. This condition is more common in older women but can occur in younger individuals as well. Given the symptoms, it would be prudent to have this condition ruled out by a healthcare professional.
5. Other Skin Conditions: Other dermatological conditions, such as eczema or psoriasis, could also manifest in the nipple area, causing itchiness and changes in skin texture or color.
Given that the symptoms are localized to the right nipple and not present on the left, it is essential to monitor the situation closely. Here are some recommendations:
- Avoid Irritants: Ensure that your daughter avoids any potential irritants, such as harsh soaps or lotions, and wears breathable fabrics.
- Keep the Area Clean and Dry: Maintaining good hygiene can help prevent infections. Ensure the area is kept clean and dry.
- Seek Medical Attention: Given the symptoms, especially the darkening of the nipple and the presence of a discharge, it is advisable to consult a healthcare provider. A dermatologist or pediatrician can perform a thorough examination and may conduct tests to determine the underlying cause of the symptoms.
In summary, while itchy nipples and associated symptoms can be common during adolescence due to hormonal changes, the specific symptoms your daughter is experiencing warrant a professional evaluation to rule out any serious conditions and to provide appropriate treatment. Early intervention can lead to better outcomes and peace of mind.
