White Bumps Near the Glans: Pearly Penile Papules vs. STIs - Urology

Sudden appearance of multiple white spots near the glans?

Hello doctor, I would like to ask a question.
I only had a circumcision when I reached college, and during high school, I noticed a white ring around the groove near the glans.
After researching online, I found that it might be pearly penile papules.
At that time, I wasn't worried because I hadn't had sexual relations.
However, after the circumcision, I had sexual encounters with five different partners.
I did not use a condom during oral sex, but I did use condoms during intercourse.
Recently, I discovered that similar pearly papules have appeared in the groove behind the glans.
Is it possible for them to develop even after circumcision, or could this be a sign of genital warts due to having multiple sexual partners? It's a bit difficult for me to bring up the topic of sexually transmitted infections when visiting the hospital.

Hello: It's hard to say.
Based on personal experience, pearly penile papules can sometimes be prominent and other times less noticeable.
If they increase in number or size, it's best to consult a physician.
Doctors see many cases daily, so there's no need to worry too much.
Wishing you good health.

When it comes to the appearance of white bumps near the glans, it is essential to differentiate between common benign conditions, such as pearly penile papules (PPP), and sexually transmitted infections (STIs) like genital warts (caused by the human papillomavirus, HPV).
Pearly penile papules are small, dome-shaped, white or flesh-colored bumps that typically appear in a single or double row around the corona of the glans. They are a normal anatomical variant and are not associated with any disease or infection. PPP is particularly common in uncircumcised men but can also appear in circumcised men, as you have experienced after your circumcision. These papules are harmless and do not require treatment unless they cause psychological distress.

On the other hand, genital warts, which are caused by certain strains of HPV, can also present as small bumps in the genital area. They may appear as raised, flesh-colored or white lesions and can vary in size and shape. Unlike PPP, genital warts can be contagious and are typically associated with sexual activity. If you have multiple sexual partners, the risk of contracting STIs, including HPV, increases.

Given your history of sexual activity, particularly oral sex without a condom, it is prudent to consider the possibility of STIs. While PPP is benign, the presence of new or changing lesions warrants further evaluation to rule out any infections. It is understandable to feel apprehensive about discussing sexual health concerns with a healthcare provider, but it is crucial for your health and peace of mind.

I recommend scheduling an appointment with a healthcare professional who specializes in sexual health or urology. They can perform a thorough examination and may recommend tests for STIs if deemed necessary. It is essential to be open about your symptoms and sexual history to receive appropriate care.
In summary, while pearly penile papules are harmless and can occur even after circumcision, the appearance of new bumps, especially after engaging with multiple partners, should be evaluated to rule out STIs. Early diagnosis and treatment can help manage any potential infections and alleviate concerns. Remember, sexual health is an important aspect of overall well-being, and seeking help is a responsible step towards maintaining it.
