Cauliflower
Hello, doctor.
I would like to ask about the white small bumps that have developed around the edge of my glans.
They have been present for about 12 years and encircle the entire glans.
They look very much like what you refer to as pearly penile papules.
However, I am very concerned that they might be genital warts.
Could you please explain what genital warts look like? How long can genital warts go untreated before complications arise? Where can I find images of genital warts and pearly penile papules for comparison? Thank you!
KK, 20~29 year old female. Ask Date: 2005/04/01
Dr. Cai Bingkun reply Urology
Hello! Based on your description, it seems to be a cholesteatoma.
You should be able to find images of cauliflower-like lesions and cholesteatomas in dermatology and urology textbooks.
Thank you for your inquiry, and I wish you good health and happiness!

More Info
Pearly penile papules (PPP) and genital warts are two distinct conditions that can appear on the genital area, and understanding their differences is crucial for proper diagnosis and management.
Pearly Penile Papules (PPP)
Pearly penile papules are small, dome-shaped, white or skin-colored bumps that typically appear around the corona (the ridge) of the glans (the head) of the penis. They are a normal anatomical variant and are not caused by any infection or sexually transmitted disease. PPP is most commonly seen in young adult males and is considered a benign condition.
Characteristics of PPP:
- Appearance: Small, smooth, and round bumps that are usually white or flesh-colored.
- Location: Found in a single row around the glans of the penis.
- Symptoms: Generally asymptomatic, meaning they do not cause pain, itching, or discomfort.
- Duration: They are a permanent feature once they appear and do not change over time.
- Diagnosis: Diagnosis is typically made through visual examination by a healthcare provider. No treatment is necessary unless the individual desires removal for cosmetic reasons.
Genital Warts
Genital warts, on the other hand, are caused by the human papillomavirus (HPV), a sexually transmitted infection. They can appear as small, flesh-colored or gray growths that may be raised or flat, and they can cluster together to form a cauliflower-like shape.
Characteristics of Genital Warts:
- Appearance: Can vary in size and shape; they may be flat or raised and can appear as single lesions or in clusters resembling cauliflower.
- Location: Can occur on the genital area, including the vulva, vagina, cervix, anus, and the penis.
- Symptoms: May cause itching, discomfort, or bleeding, especially if they are irritated.
- Duration: Genital warts can appear weeks to months after exposure to HPV and may resolve on their own, but they can also persist or recur.
- Diagnosis: Diagnosis is made through visual examination, and sometimes a biopsy may be performed to rule out other conditions.
Key Differences
1. Cause: PPP is a normal anatomical variant, while genital warts are caused by HPV.
2. Appearance: PPP are small, smooth, and dome-shaped, while genital warts can vary in size and have a rough, cauliflower-like appearance.
3. Symptoms: PPP are usually asymptomatic, whereas genital warts can cause discomfort or irritation.
4. Treatment: PPP typically do not require treatment, while genital warts may need to be treated with topical medications, cryotherapy, or surgical removal.
Concerns About Delay in Treatment
If you suspect that you have genital warts, it is important to seek medical advice. While PPP is harmless, genital warts can lead to complications, including the potential for certain types of HPV to increase the risk of cervical cancer in women. Early diagnosis and treatment can help manage symptoms and reduce the risk of transmission to sexual partners.
Visual Resources
For visual references, many reputable medical websites provide images of both conditions. Websites such as the American Academy of Dermatology or the Centers for Disease Control and Prevention (CDC) often have educational resources that include images and descriptions of various skin conditions, including PPP and genital warts.
In conclusion, while both conditions may appear similar at first glance, they have distinct characteristics and implications. If you are concerned about the growths you are experiencing, it is advisable to consult a healthcare provider for an accurate diagnosis and appropriate management.
