Problems with Rubber Band Ligation for Internal Hemorrhoids
Hello, Dr.
Yu.
In recent years, I have developed hemorrhoids.
Two days ago, I experienced severe bleeding due to diarrhea.
On 10/26, when I visited the doctor, I was informed that I could undergo hemorrhoid ligation surgery, which I did.
There were no major issues before or after the surgery, aside from a foreign body sensation and mild pain.
The next day, on 10/27, I returned to work and had a bowel movement.
The process was relatively easy (with some pain), but I am unsure if the pain is due to standing and frequent walking, as I sometimes experience severe pain from anal contractions.
Usually, I only feel a foreign body sensation and mild pain, but there is also a feeling of tightness in the anal area.
Is this normal, or could it be due to excessive tension? I later noticed difficulty urinating and an inability to pass urine smoothly (with anal pain).
Additionally, I would like to ask if anal contractions are a normal phenomenon.
I often experience involuntary contractions; could this lead to worsening of the wound or hemorrhoids? The contractions are accompanied by intense pain.
How should I manage this or seek further medical attention? This is my condition on the first day post-surgery.
Thank you, doctor.

Post-surgery pain and complications following hemorrhoid banding can be a significant concern for many patients. It is essential to understand the normal healing process, potential complications, and when to seek further medical attention.
After hemorrhoid banding, it is common to experience some discomfort, which may include a sensation of fullness or pressure in the anal area, mild to moderate pain, and occasional bleeding. These symptoms are generally expected as the body heals from the procedure. However, the intensity and duration of pain can vary significantly among individuals.
In your case, experiencing severe pain during bowel movements, especially after standing or walking, could be attributed to several factors. First, the act of straining during a bowel movement can exacerbate pain, particularly if the anal area is still healing from the banding procedure. It is crucial to maintain soft stools to minimize straining. This can be achieved through dietary adjustments, such as increasing fiber intake and staying well-hydrated. Over-the-counter stool softeners may also be beneficial.
The sensation of tightness or spasms in the anal area, which you described as "anal contraction," can occur post-surgery. This is often a reflex response to pain or discomfort and can lead to a cycle of increased pain and tension. While some degree of contraction is normal, excessive or involuntary spasms can be problematic. If these contractions are accompanied by severe pain, it may indicate that the area is not healing properly or that there is an underlying issue, such as an infection or a complication from the procedure.
Regarding your urinary symptoms, difficulty urinating can sometimes occur after hemorrhoid surgery due to pain or anxiety about the pain associated with bowel movements. The pelvic floor muscles can become tense, leading to urinary retention. If this persists, it is essential to consult your healthcare provider, as they may need to evaluate for any complications related to the surgery or other underlying conditions.
In summary, while some pain and discomfort are normal after hemorrhoid banding, severe pain, especially if it is worsening or accompanied by other symptoms like significant bleeding or difficulty urinating, warrants further evaluation. It is advisable to reach out to your surgeon or healthcare provider to discuss your symptoms in detail. They may recommend pain management strategies, such as medications or physical therapy, to help alleviate muscle tension and improve your recovery experience.
In the meantime, focus on maintaining a diet rich in fiber, staying hydrated, and avoiding straining during bowel movements. Gentle warm baths may also provide relief and promote relaxation of the anal muscles. Remember, open communication with your healthcare provider is key to ensuring a smooth recovery and addressing any concerns you may have.
