Hello doctor, what is this condition?

Hello, Doctor.
I have pain in my right leg, specifically a point in my buttock that sometimes hurts when I walk, while at other times it feels okay.
Pressing on that point in my buttock causes mild pain.
When I bend over, I feel the bones in my buttock moving inward, and when I stand up, they return to their original position.
When lying down with my legs straight, I can also feel movement in the hip bones during certain actions.
There are tender points on the inner thigh near the groin and on the inner side of the knee, which are quite sore when pressed.
The outer side near the buttock is also tender, but the front and back of the thigh do not have much soreness or pain.
My calf feels tight, almost cramping at times.
Additionally, sometimes my right foot has a strange, indescribable sensation that leads to insomnia.
Doctor, what could this condition be? Does your hospital offer detailed consultations, relevant examinations, and treatments? If so, which department should I visit? Thank you for your response.

Hello: Based on your age, unless there is a significant trauma or major issue, it is unlikely that you would experience hip pain.
Given your symptoms, it seems related to tightness in the gluteal muscles.
Further examination at the outpatient clinic is necessary to provide additional recommendations.
Changhua Hospital cares about your health.

More Info

Understanding hip pain can be quite complex, as it often involves a variety of symptoms, potential diagnoses, and treatment options. Based on your description, it seems that you are experiencing discomfort in the right hip area, with pain that varies in intensity during different activities. You also mentioned specific points of tenderness in the hip, thigh, and knee regions, as well as sensations of tightness and potential cramping in the calf.

Symptoms and Possible Diagnoses
1. Localized Pain: The pain you feel in the hip, especially when pressing on specific points, could indicate issues such as muscle strain, tendonitis, or inflammation of the bursae (bursitis). The hip area is home to several muscles, including the gluteal muscles and the iliopsoas, which can become tight or inflamed due to overuse or improper posture.

2. Movement-Related Discomfort: The sensation of the hip bone moving or shifting during certain movements could suggest hip joint instability or issues with the surrounding soft tissues. This could be related to conditions such as hip impingement or labral tears, which can cause pain and a feeling of instability.

3. Referred Pain: The discomfort you feel in the inner thigh and near the knee could be referred pain from the hip joint or surrounding muscles. Conditions like piriformis syndrome, where the piriformis muscle irritates the sciatic nerve, can lead to pain radiating down the leg.

4. Muscle Cramping: The cramping sensation in your calf may be related to muscle fatigue or dehydration, but it could also be a sign of nerve involvement or vascular issues, especially if it occurs frequently or with exertion.

Diagnostic Approach
Given the complexity of your symptoms, a thorough evaluation is essential. Here are some steps you can take:
1. Consultation: Start with a visit to an orthopedic specialist or a physical medicine and rehabilitation physician. They can perform a detailed physical examination and assess your range of motion, strength, and any specific areas of tenderness.

2. Imaging Studies: Depending on the initial assessment, your doctor may recommend imaging studies such as X-rays or MRI to evaluate the bones and soft tissues of the hip joint. These tests can help identify any structural abnormalities, such as fractures, arthritis, or soft tissue injuries.

3. Physical Therapy: If no significant structural issues are found, a referral to a physical therapist may be beneficial. They can develop a tailored exercise program to strengthen the hip muscles, improve flexibility, and address any postural issues that may be contributing to your pain.

Treatment Options
1. Conservative Management: Initially, treatment may focus on conservative measures such as rest, ice application, and over-the-counter pain relief medications (e.g., NSAIDs) to reduce inflammation and pain.

2. Physical Therapy: Engaging in physical therapy can help improve muscle strength and flexibility, which may alleviate some of the discomfort you are experiencing. Therapists may also employ techniques such as ultrasound therapy or electrical stimulation to aid in pain relief.

3. Injections: In some cases, corticosteroid injections may be recommended to reduce inflammation in the hip joint or surrounding tissues.

4. Surgical Options: If conservative treatments fail and significant structural issues are identified, surgical options may be considered. This could include arthroscopy to repair any tears or to address impingement.

Conclusion
In summary, your symptoms suggest a multifaceted issue involving the hip joint and surrounding musculature. A comprehensive evaluation by a specialist is crucial for accurate diagnosis and effective treatment. I recommend scheduling an appointment with an orthopedic or rehabilitation physician to discuss your symptoms in detail and explore the appropriate diagnostic tests and treatment options. Remember, early intervention can often lead to better outcomes, so seeking help sooner rather than later is advisable.
