Balanitis, nearing recovery, with wrinkling of the glans and foreskin?

Hello Doctor, I would like to ask some questions.
I have phimosis, but I have no issues with retraction.
I clean daily, but in early October, I noticed a red patch on the glans, which gradually expanded to cover the entire glans and foreskin.
Then, the foreskin and glans started to peel.
At its worst on October 21, I developed ulcers that were painful to touch.
I consulted a dermatologist and a urologist, took antibiotics, and applied ointments, and the wounds have healed.
My last consultation was on November 12, where the doctor said I was better but prescribed a combination ointment containing steroids and antifungals.
However, I still feel something is off; it hasn't completely healed.
Although there is no pain, itching, ulcers, or swelling now, the glans and foreskin remain persistently red.
I mostly keep the glans exposed to air to stay dry, but the skin on the glans and foreskin is excessively wrinkled, even more than an elderly person's skin.
When erect, it appears smooth but feels tight, and after erection, it becomes very wrinkled.
When touching it in this wrinkled state, it feels like there is a thin, transparent membrane that seems fragile and could break easily, resembling the condition before my last peeling episode.
Therefore, I would like to ask the doctor: 1.
Is the redness of the glans possibly caused by the medication (I have been applying the ointment continuously for about a month)? 2.
Are the numerous wrinkles on the glans a sign of impending peeling, is this normal, or does it indicate that it hasn't fully healed yet, or is it the final stage of the treatment for balanitis? I currently have no pain or itching.
Thank you for taking the time to address my questions.
I greatly appreciate it.

Dr. Du Shixi reply Urology

Hello: 1) Glans redness can vary; some cases are due to an active infection, while others may be from a chronic infection that hasn't completely resolved but is gradually improving.
2) The presence of many wrinkles on the glans is a result of prolonged inflammation, which has caused changes in the skin.
If inflammation ceases, improvement is possible.
Currently, it is important to maintain cleanliness and dryness, avoiding moisture.
Wear loose-fitting underwear, and once the topical ointment has been applied and the condition improves, it may no longer be necessary to continue its use.
Practice safe sex, and if there is a family history of diabetes, it may be advisable to check blood sugar levels.
Wishing you good health.

Understanding glans inflammation, particularly in the context of your symptoms, is crucial for ensuring proper healing and addressing any lingering concerns. Based on your description, it seems you have experienced a significant episode of inflammation and irritation of the glans and foreskin, which can be quite distressing.

1. Redness of the Glans: The persistent redness of the glans could be attributed to several factors. While you mentioned that you have been using a topical steroid and antifungal cream, it is important to note that prolonged use of topical steroids can sometimes lead to skin thinning and increased sensitivity, which may manifest as redness. Additionally, if there was an underlying infection or inflammatory condition, the redness could be a residual effect of that condition. Since you have already seen specialists and have been treated, it is essential to monitor the area for any changes. If the redness persists or worsens, it would be advisable to follow up with your healthcare provider for further evaluation.

2. Wrinkling of the Glans: The wrinkling of the glans, especially when flaccid, can be a normal physiological response. The skin on the glans is quite delicate and can appear wrinkled when it is not fully engorged with blood. This is particularly common after episodes of inflammation or irritation. The sensation of a thin, transparent membrane could be related to the healing process, where the skin is still recovering from the previous inflammation and may appear more fragile. It is not uncommon for the skin to have a different texture during the healing phase, and as long as there is no pain, discharge, or other concerning symptoms, this may simply be part of the recovery.

3. Healing Process: The healing process for glans inflammation can vary significantly from person to person. It is not unusual for the skin to take time to return to its normal state after an inflammatory episode. The fact that you are no longer experiencing pain or discomfort is a positive sign. However, the presence of ongoing symptoms like redness and wrinkling suggests that the area is still in the process of healing. It is essential to continue practicing good hygiene and to avoid any irritants that could exacerbate the condition.

4. When to Seek Further Help: If you notice any new symptoms, such as increased redness, pain, discharge, or if the condition does not improve over the next few weeks, it would be prudent to consult your healthcare provider again. They may consider alternative treatments or further investigations to rule out any underlying conditions.

In summary, while the redness and wrinkling of the glans can be concerning, they may be part of the normal healing process following inflammation. Continued observation and good hygiene practices are key. If symptoms persist or worsen, seeking further medical advice is recommended to ensure complete recovery.
