Drug interactions

Hello, we recently purchased Xunli Ning for our parents to take, but since both of them are on some chronic medications, we are concerned about the possibility of drug interactions.
Here are the medications and supplements they are currently taking:
1.
My father is currently taking a lipid-lowering medication, Guanzhitong, every other day as per his doctor's instructions, along with allergy medication, Ruo Ta Pian, Pei Ni An, and An Si Ping.
His other supplements include Xunli Ning (three times a day, one capsule each time), fish oil (one capsule a day containing 366mg EPA and 135mg DHA), calcium tablets, a multivitamin, lutein, and anthocyanins.

2.
My mother, who has a history of acute hepatitis, is continuously taking Beileke.
Her other supplements are the same as my father's, plus sesame lignan.

We found online that fish oil and ginkgo biloba seem to have anticoagulant effects, and we are not very familiar with the interactions of other supplements.
Therefore, we would like to know if there are any specific medications or supplements that should not be taken together and need to be completely discontinued, or if they can simply be taken at different times.
Additionally, if dental treatment is to be performed, are there any supplements that should be avoided to prevent excessive bleeding? Thank you for your assistance!

Ms. Li, 40~49 year old female. Ask Date: 2018/11/29

Dr. Liao Lingmei reply Pharmacology

Hello: After reviewing the aforementioned medications, dietary supplements, and nutritional products (Rosuvastatin, Loratadine/Loratadine+Pseudoephedrine, Cyproheptadine, Azathioprine, Ginkgo biloba, Fish oil, Calcium, Multi-vitamin, Lutein, Anthocyanin, Baraclude), no reports of interactions among them were found.
However, each of them may have interactions with specific foods, which you were likely informed about when receiving the medications or making purchases.
Wishing you health and happiness!

More Info

Understanding drug interactions is crucial, especially for individuals taking multiple medications and supplements, as is the case with your parents. Chronic medication users often face the risk of adverse interactions, which can lead to reduced efficacy of medications or increased side effects. Here’s a comprehensive overview of the potential interactions and safety tips for your parents' medication regimen.

1. Understanding the Medications and Supplements
Your father is taking a combination of medications for cholesterol management (冠脂妥), allergy (柔它錠), and anxiety (配你安, 安思平), along with several supplements, including循利寧, fish oil, calcium, multivitamins, lutein, and anthocyanins. Your mother is on 贝乐克 for her liver condition, along with similar supplements and sesame lignans.

2. Potential Drug Interactions
#
Fish Oil and Ginkgo Biloba
Both fish oil and ginkgo biloba have blood-thinning properties. If your parents are taking any anticoagulants or antiplatelet medications, such as aspirin or other blood thinners, combining these could increase the risk of bleeding. It’s essential to monitor for signs of excessive bleeding, such as unusual bruising or prolonged bleeding from cuts.

#
Calcium and Certain Medications
Calcium supplements can interfere with the absorption of certain medications, particularly antibiotics and some blood pressure medications. It’s advisable to space calcium supplements at least two hours apart from other medications to minimize this interaction.

#
Liver Health Considerations
Given your mother’s history of acute hepatitis, it’s crucial to be cautious with any supplements that could affect liver function. Some herbal supplements can be hepatotoxic, so it’s best to consult with a healthcare provider before continuing any new supplements.

3. Safety Tips for Chronic Medication Users
- Consult Healthcare Providers: Always discuss any new medications or supplements with a healthcare provider, especially for individuals with chronic conditions. This includes informing the doctor about all medications and supplements currently being taken.

- Monitor for Side Effects: Encourage your parents to be vigilant about any new symptoms or side effects after starting a new medication or supplement. Keeping a journal of any changes can help healthcare providers make informed decisions.

- Timing of Doses: If certain supplements or medications are known to interact, consider staggering their intake. For example, if taking calcium, it should be taken at a different time than medications that may be affected by it.

- Avoid Certain Supplements Before Dental Procedures: Before dental treatments, it’s advisable to avoid supplements that can increase bleeding risk, such as fish oil and ginkgo biloba. It’s best to consult with the dentist regarding which supplements should be paused prior to any procedures.

4. Specific Recommendations for Your Parents
- For Your Father: Ensure that he is not taking any additional blood thinners without medical advice. Monitor for any signs of bleeding, especially if he is taking fish oil or ginkgo biloba.

- For Your Mother: Given her liver condition, it’s essential to avoid any supplements that could potentially harm the liver. Discuss with her healthcare provider about the safety of continuing the current regimen, especially the sesame lignans.

5. Conclusion
Managing multiple medications and supplements requires careful consideration to avoid interactions that could compromise health. Regular consultations with healthcare providers, monitoring for side effects, and being informed about the medications and supplements being taken are key strategies for ensuring safety. If there are any concerns or uncertainties, it’s always best to reach out to a pharmacist or healthcare professional for personalized advice.
