Post-Surgery Concerns: What to Do About Stitches After Hemorrhoid Surgery - Surgery

Anal gland or anal sac?

Hello Doctor, I underwent hemorrhoid surgery on December 12th, and today, December 18th, I noticed that a stitch has come out after having a bowel movement.
The follow-up appointment with the hospital is not yet due.
Is this situation normal? It seems that one of the stitches has come apart a little bit.
The pain is not very severe.
Should I wait for my follow-up appointment for the doctor to cut the stitch? However, I read online that stitches usually fall out in 1 to 2 weeks.
Why did it come out just a few days after surgery? Could this be related to straining during bowel movements?

Miss Huang, 20~29 year old female. Ask Date: 2018/12/18

Dr. Xu Kaixi reply Surgery

The sutures used for hemorrhoid surgery may typically fall out during bowel movements or straining, and it is normal for the wound to occasionally separate slightly as a result.
Doctors usually assess the situation and may remove any visible remaining sutures around two to three weeks post-surgery.

After undergoing hemorrhoid surgery, it is not uncommon for patients to have concerns about the healing process, particularly regarding stitches and any visible suture ends. In your case, noticing a stitch end protruding from the anal area just six days post-surgery can understandably cause anxiety.
Firstly, it is essential to understand that during hemorrhoid surgery, the surgeon typically uses absorbable sutures to close the surgical site. These sutures are designed to dissolve over time, usually within one to two weeks, depending on the type of material used and the individual’s healing process. However, it is not unusual for some patients to notice the ends of these sutures before they have fully absorbed, especially if the surgical site is under tension or if there has been any straining during bowel movements.

The fact that you are experiencing some separation at one of the suture sites is something to monitor. While minor separation can occur, it is crucial to ensure that the wound is not becoming infected or that there is no significant opening that could lead to complications. Pain levels can vary; if you are experiencing only mild discomfort, that is generally a positive sign, but it is still important to keep an eye on any changes.

Regarding your question about whether you should wait for your follow-up appointment to have the stitch trimmed, it is advisable to consult your surgeon or the medical team that performed your procedure. They can provide specific guidance based on your individual situation. If the stitch is causing significant discomfort or if you notice any signs of infection (such as increased redness, swelling, or discharge), you should seek medical attention sooner rather than later.

As for the relationship between straining during bowel movements and the stitch issue, it is indeed possible. Straining can put additional pressure on the surgical site, potentially leading to complications such as increased pain or even the premature loosening of sutures. It is crucial to maintain soft stools to minimize straining. This can often be achieved through dietary adjustments, such as increasing fiber intake and staying well-hydrated. Over-the-counter stool softeners may also be beneficial, but it is best to discuss this with your healthcare provider.

In summary, while it is not uncommon to notice stitch ends shortly after hemorrhoid surgery, it is essential to monitor the situation closely. Keep the area clean and dry, avoid straining during bowel movements, and consult your surgeon if you have any concerns or if the situation changes. Your follow-up appointment will be an excellent opportunity to address these issues and ensure that your healing process is on track. Remember, open communication with your healthcare provider is key to a smooth recovery.
