Post-ejaculation swelling
Hello doctor, last month while I was masturbating, I stimulated the glans particularly, and during ejaculation, my penis suddenly swelled, feeling like it was about to burst.
I calmed down immediately, and the sensation gradually subsided.
After that, I felt dizzy.
Two weeks later, when I masturbated again, the same symptoms occurred.
Recently, I've noticed a prominent vein on my penis.
I usually masturbate once a week and go to bed around midnight.
The semen I ejaculate does not contain blood.
I have not had a circumcision, and the frenulum feels a bit tight.
I'm not sure what these symptoms mean.
I also have slightly elevated blood pressure, around 130/80, and I'm wondering if that could be related.

Dr. Du Mingyi reply Urology
It is likely a normal physiological phenomenon that does not require special attention.
If there is discomfort, resting for a while should help.

Understanding post-ejaculation swelling and the associated symptoms can be concerning, especially when accompanied by unusual sensations or changes in the anatomy of the penis. Based on your description, it seems that you are experiencing a few distinct phenomena after ejaculation, including swelling of the penis, a feeling of pressure, and the appearance of a prominent vein.
Firstly, the swelling of the penis post-ejaculation can be attributed to several factors. During sexual arousal and ejaculation, blood flow to the penis increases significantly, leading to an erection. This process is facilitated by the engorgement of the erectile tissues (corpora cavernosa and corpus spongiosum) with blood. If there is excessive stimulation or a particularly vigorous activity, it can lead to temporary swelling or engorgement that may feel pronounced. This is generally a normal physiological response, but if it is accompanied by discomfort or unusual sensations, it may warrant further investigation.
The feeling of pressure or a sensation that the penis is being "stretched" could be related to the intensity of the stimulation you experienced. If you are stimulating the glans (the head of the penis) more vigorously than usual, it could lead to heightened sensitivity and a more pronounced response during ejaculation. However, if this sensation is accompanied by pain or discomfort, it may indicate that the stimulation is too aggressive or that there could be an underlying issue, such as inflammation or irritation.
Regarding the appearance of a prominent vein, this can be a normal anatomical variation, especially if you are noticing it more after periods of increased blood flow during sexual arousal or masturbation. However, if the vein appears suddenly and is accompanied by pain, swelling, or discoloration, it could indicate a condition known as a thrombosed vein or even a varicocele, which is an enlargement of the veins within the scrotum. These conditions can sometimes cause discomfort and may require medical evaluation.
You mentioned experiencing dizziness after these episodes, which is less common and could be related to several factors, including changes in blood pressure or heart rate during sexual arousal and ejaculation. It’s important to monitor your symptoms closely. If you find that the dizziness persists or worsens, it would be advisable to consult a healthcare professional.
As for your blood pressure, a reading of 130/80 mmHg is considered to be at the upper end of normal. While this may not directly cause the symptoms you are experiencing, maintaining a healthy lifestyle, including regular exercise, a balanced diet, and stress management, can help manage blood pressure and overall cardiovascular health.
Lastly, the tightness of the frenulum (the band of tissue on the underside of the penis) can sometimes lead to discomfort during sexual activity, especially if it restricts movement. If you find that this is causing pain or contributing to your symptoms, discussing this with a healthcare provider may be beneficial. They can provide options for treatment, which may include a minor surgical procedure if necessary.
In summary, while some of the symptoms you are experiencing may be within the realm of normal physiological responses, the combination of swelling, pressure, and dizziness should be evaluated by a healthcare professional to rule out any underlying conditions. It’s always better to err on the side of caution when it comes to health concerns, especially those involving sexual health.
