Swelling above the testicles during ejaculation and urinary issues?
Hello, Dr.
Lee.
I currently engage in DIY at least once a day (up to twice a day), but sometimes when I'm close to ejaculation, I notice a sudden swelling of a lump at the junction of my thigh and penis (not the testicles, but above the testicles, on the left side of the penis when viewed from above).
When I press on it, it doesn't swell anymore and seems to be movable.
I'm not sure what this is, but I'm a bit worried because I can vaguely feel it swelling before ejaculation, although it's not severe.
It only becomes more pronounced and uncomfortable as I approach ejaculation, which is very uncomfortable, so I press on that "junction" to prevent it from swelling.
At that point, I feel nothing; it’s neither painful nor uncomfortable.
However, I'm concerned and really want to know what this is and why it swells.
Before ejaculation, it doesn't swell; if there is any swelling, it's very minor and barely noticeable.
It only becomes significant as I near ejaculation.
I’ve searched for information but haven’t found anything relevant.
Additionally, I usually experience some symptoms of frequent urination, meaning that shortly after I urinate, I feel the urge to go again.
At its worst, I would leave the restroom and immediately feel a very strong urge to urinate, but then I couldn’t urinate at all, which was very painful.
Fortunately, it hasn't been that severe recently, and I haven't experienced it this year.
Could this be related to the swelling issue? I smoke regularly, and I know I might be engaging in DIY too frequently.
Due to my job, I often sit for long periods, either at a computer or a desk, and I rarely exercise.
Could this also be related? I'm very concerned about the swelling during ejaculation and want to know what is causing it.
Regarding my urination issues, I would like to understand what symptoms these are and what condition they might indicate.
I've heard that benign prostatic hyperplasia is something that only middle-aged men experience; could this be a possibility for me as well?
I kindly ask for your insights on the swelling during ejaculation and the urination issues.
Thank you very much.

A physician must personally examine a patient before administering treatment, prescribing medication, or providing a diagnosis.
It is not prostate enlargement, but it resembles prostatitis.
Poor lifestyle habits may be related.
Based on your description, the symptoms you mentioned are not specific enough for a diagnosis.
It is advisable to consult a urologist for an examination and discuss your concerns with the physician.
Try to relax and avoid fixating on that area; do not use it as an outlet for seeking stimulation, as misuse can lead to various issues.
Excessive sexual activity may indicate a loss of balance in life.
Overall, this is not beneficial.
It is recommended to rest well and observe your physical and mental health; some issues may resolve themselves.

Hello! Thank you for your detailed question regarding the testicular swelling during ejaculation and your urination issues. It’s important to address these concerns as they can significantly affect your quality of life.
Firstly, the swelling you are experiencing during ejaculation could be related to a few different anatomical structures in the area. The area above the testicles that you describe could be the epididymis or the spermatic cord, which contains blood vessels, nerves, and the vas deferens. When you are nearing ejaculation, there is increased blood flow and pressure in the pelvic region, which can lead to temporary swelling or engorgement of these structures. This is generally a normal physiological response, but if it is accompanied by discomfort or pain, it may warrant further investigation.
The fact that the swelling subsides when you apply pressure suggests that it may be related to increased blood flow or possibly a small hernia or varicocele (enlargement of veins within the scrotum). Varicoceles are quite common and can sometimes cause discomfort, especially during activities that increase intra-abdominal pressure, such as ejaculation or heavy lifting. If the swelling is persistent or worsening, it would be advisable to consult a urologist for a physical examination and possibly an ultrasound to rule out any underlying conditions.
Regarding your urinary symptoms, frequent urination and the sensation of incomplete bladder emptying can be associated with several conditions. While benign prostatic hyperplasia (BPH) is more common in older men, younger individuals can also experience similar symptoms due to various factors, including lifestyle and habits. Factors such as excessive caffeine or alcohol intake, smoking, and a sedentary lifestyle can contribute to urinary urgency and frequency. Additionally, anxiety and stress can exacerbate these symptoms.
Your description of feeling a strong urge to urinate shortly after using the restroom, along with the discomfort of not being able to void completely, could indicate a condition known as bladder irritability or overactive bladder. This can be influenced by lifestyle factors, including prolonged sitting and lack of physical activity, which can affect pelvic floor muscle tone and bladder function.
It’s also worth noting that smoking can have a negative impact on overall vascular health, which may contribute to both your urinary and sexual health issues. Quitting smoking and incorporating regular physical activity into your routine could potentially improve your symptoms.
In summary, the swelling during ejaculation may be a normal physiological response, but if it causes discomfort or is accompanied by other concerning symptoms, it’s best to seek medical advice. Your urinary symptoms could be related to lifestyle factors, but they may also warrant further evaluation to rule out any underlying conditions. A consultation with a urologist would be beneficial for a comprehensive assessment and tailored advice based on your specific situation.
Please take care of your health, and don’t hesitate to reach out to a healthcare professional for further evaluation and management of your symptoms.
