Is there blood in the semen?

Hello, Dr.
Hsu.

1.
I have been experiencing testicular pain for some time, with noticeable swelling in the right testicle.
It feels slightly painful when touched.
I refrained from masturbation for a week, but suddenly felt an urge.
After masturbating, I noticed that the latter part of my semen had blood in it.
What could be the reason for this?
2.
Why does the testicular swelling and pain sometimes lead to an erection, while at other times, despite feeling the urge to masturbate, I am unable to achieve an erection?
Thank you for your assistance, Dr.
Hsu.

Lin, 20~29 year old female. Ask Date: 2011/04/11

Hello, if you are experiencing testicular swelling, you should consider the following: 1.
Epididymitis or Orchitis 2.
Testicular tumor.
If there is blood in the semen, it is advisable to go to the hospital for an examination to determine whether it is due to inflammation or another issue causing the bleeding.
Testicular pain, which may radiate to the penis, could be due to stimulation of the sexual nerves, leading to phenomena such as erection.
(Note: Bladder distension can also trigger an erection.) If you have any further questions, feel free to ask for more discussion and research.
This response is from Dr.
Hsu Wei-Kai, Chief of Urology, Taitung Hospital, Department of Health, Executive Yuan.

Understanding blood in semen and associated testicular pain can be concerning, and it's important to address these symptoms with a comprehensive approach.
1. Blood in Semen (Hematospermia): The presence of blood in semen, known as hematospermia, can arise from various causes. Common reasons include inflammation or infection of the prostate (prostatitis), seminal vesicles, or urethra. Other potential causes could be trauma to the genital area, benign prostatic hyperplasia (BPH), or even more serious conditions like tumors or vascular issues. In your case, the occurrence of blood in the semen following testicular pain could suggest an underlying issue that may need further investigation.

2. Testicular Pain and Swelling: The swelling and pain in the right testicle could be due to several factors, including epididymitis (inflammation of the epididymis), testicular torsion, or a hernia. Epididymitis is often associated with infections, which can also lead to hematospermia. The pain you experience during erections or while attempting to engage in sexual activity could be linked to the inflammation or swelling affecting the nerves and blood vessels in the area.
3. Variability in Symptoms: The fluctuation in your symptoms—where you experience pain that can sometimes lead to an erection and other times does not—can be attributed to several factors. Pain can inhibit sexual arousal and function, leading to difficulties in achieving or maintaining an erection. Conversely, when the pain subsides, sexual desire may return, but the underlying issue may still be present, causing discomfort during arousal.

Recommendations:
- Medical Evaluation: It is crucial to seek a thorough evaluation from a healthcare professional, preferably a urologist. They may recommend a physical examination, ultrasound imaging of the scrotum, and possibly laboratory tests to check for infections or other underlying conditions.

- Avoiding Irritation: Until you have a clearer understanding of the cause of your symptoms, it may be wise to avoid activities that could exacerbate the pain, such as vigorous sexual activity or heavy lifting.

- Hydration and Health: Ensure you are well-hydrated and maintain a healthy lifestyle, as overall health can impact sexual function and recovery from infections.

- Follow-Up: If you experience worsening symptoms, such as increased pain, fever, or changes in urination, seek immediate medical attention.
In summary, while the presence of blood in semen and testicular pain can be alarming, they are often treatable conditions. Early intervention and proper medical guidance are key to addressing these symptoms effectively.
