Premature Ejaculation: Tips for Lasting Longer - Urology

Regarding premature ejaculation?

Hello doctor, I am 25 years old with a BMI of 21 and no bad habits.
I masturbate once every 1-2 weeks.
Two years ago, I discovered that I had phimosis and have been applying medication; now the glans can be exposed.
My sexual experience is limited, and I currently use a masturbator (simulating sexual positions).
I want to try to improve my stamina, but I find that I cannot control the glans when I feel close to ejaculation (while watching adult films).
At the moment of climax, the glans becomes excessively sensitive (extremely aroused), making it hard to control.
I feel that when I simply masturbate, this issue is not present.
However, when I thrust back and forth, it is very difficult to hold back (offensive mode).
If I use the masturbator and move my hand back and forth, I can last a bit longer (defensive mode).
What should I do? Thank you.

Hello: There is a difference between your pattern and actual sexual behavior, and it may only be possible to determine if you have premature ejaculation during real sexual intercourse.
There's no need to rush.
Additionally, avoid excessive masturbation, as it can lead to fatigue.
Pay attention to your nutrition, sleep, and exercise.
Wishing you good health.

Premature ejaculation (PE) is a common concern among men, and it can be particularly frustrating for those who want to enhance their sexual performance and satisfaction. Based on your description, it seems that you are experiencing difficulty in controlling your ejaculation during sexual arousal, especially when using a simulated sexual experience like a sex toy. Here are some tips and strategies that may help you last longer during sexual activity:
1. Understanding Sensitivity: It’s important to recognize that heightened sensitivity in the glans (the head of the penis) can contribute to premature ejaculation. This sensitivity can be exacerbated by arousal levels, especially when watching pornography or engaging in sexual fantasies. You might consider desensitizing creams or sprays that can help reduce sensitivity temporarily. These products typically contain a mild anesthetic that can help you last longer.

2. Pelvic Floor Exercises: Strengthening your pelvic floor muscles can improve your control over ejaculation. Kegel exercises, which involve contracting and relaxing the muscles that control urination, can be beneficial. To perform Kegel exercises, identify the right muscles by stopping urination midstream. Once you identify them, practice contracting these muscles for a few seconds and then relaxing them. Aim for three sets of 10-15 repetitions daily.

3. Mindfulness and Relaxation Techniques: Anxiety and stress can exacerbate PE. Practicing mindfulness, deep breathing, or other relaxation techniques can help you stay calm and focused during sexual activity. This can also help you become more aware of your body’s sensations and responses, allowing you to better manage your arousal levels.

4. The Start-Stop Technique: This technique involves stimulating the penis until you feel close to ejaculation, then stopping all stimulation until the urge decreases. This can help you learn to recognize the sensations that lead to ejaculation and improve your control over time. You can practice this technique during masturbation to build confidence.

5. The Squeeze Technique: Similar to the start-stop method, the squeeze technique involves applying pressure to the base of the penis when you feel close to ejaculation. This can help reduce the urge to ejaculate and allow you to regain control. You can practice this technique alone or with a partner.

6. Gradual Exposure: Since you mentioned that using a sex toy allows you to last longer than during penetrative sex, consider gradually transitioning from masturbation to sexual activity with a partner. Start with less stimulating activities and gradually increase the intensity as you become more comfortable.

7. Communication with Your Partner: If you are engaging in sexual activity with a partner, open communication is key. Discussing your concerns and preferences can help reduce performance anxiety and create a more relaxed environment.

8. Professional Help: If these strategies do not yield satisfactory results, consider consulting a healthcare professional or a sex therapist. They can provide personalized advice and may suggest treatments such as cognitive-behavioral therapy, medications, or other interventions tailored to your needs.

9. Avoiding Pornography: Since you mentioned that watching pornography increases your arousal and sensitivity, consider reducing your consumption. Pornography can create unrealistic expectations and may contribute to performance anxiety.

10. Healthy Lifestyle: Maintaining a healthy lifestyle, including regular exercise, a balanced diet, and adequate sleep, can improve your overall sexual health and performance.

In conclusion, premature ejaculation is a manageable condition, and with the right strategies and support, you can improve your control and enhance your sexual experiences. Remember that it’s a common issue, and seeking help is a positive step towards finding a solution.
