Rapid Heartbeat Sensation in the Right Chest Area - Cardiology

There is a sensation of rapid heartbeat in the right side of the chest?

Hello Doctor, my mother is about 60 years old, 152 cm tall, and weighs approximately 43 kg.
In colder weather, she experiences a sensation in the right side of her chest that feels like her heart is beating rapidly, causing discomfort.
Previously, this condition would subside within about half an hour.
She has seen doctors who have confirmed that the heart is located on the left side, so it is not a heart issue.
She has also undergone a 24-hour Holter monitor test for arrhythmia and had a chest X-ray, both of which showed no significant abnormalities.
I would like to inquire about the possible causes of this sensation of rapid heartbeat on the right side of the chest.
Should she continue to see a cardiologist or consult another specialist? Thank you for taking the time to respond.
Wishing you all the best.

Understanding the sensation of a rapid heartbeat, especially when localized to the right side of the chest, can be complex and may involve various physiological and psychological factors. In your mother's case, several important considerations arise based on her symptoms and medical history.

Firstly, it is essential to clarify that the heart is primarily located on the left side of the chest. However, sensations of heartbeat or palpitations can sometimes be perceived in areas that are not directly related to the heart itself. This phenomenon can occur due to several reasons, including but not limited to:
1. Musculoskeletal Issues: The sensation of a rapid heartbeat in the right chest area could be related to musculoskeletal problems. Conditions such as costochondritis (inflammation of the cartilage connecting the ribs to the sternum) or muscle strain can cause discomfort that may mimic heart sensations. Given your mother's age and the potential for degenerative changes, this could be a plausible explanation.

2. Anxiety and Stress: Emotional factors such as anxiety or panic attacks can lead to increased heart rate and palpitations. If your mother experiences stress or anxiety, especially in colder weather, this could contribute to her symptoms. The body’s response to stress can manifest physically, leading to sensations that may feel like a rapid heartbeat.

3. Gastroesophageal Reflux Disease (GERD): Sometimes, gastrointestinal issues can cause sensations that mimic cardiac symptoms. GERD can lead to discomfort in the chest area, and some patients report palpitations or a racing heart due to irritation of the esophagus or vagus nerve.

4. Arrhythmias: Although previous tests, including a 24-hour Holter monitor, did not reveal significant arrhythmias, it is still possible for intermittent arrhythmias to occur. These can sometimes be missed during short-term monitoring. If the sensation persists or worsens, further cardiac evaluation may be warranted.

5. Pulmonary Issues: Conditions affecting the lungs, such as pleuritis or pulmonary embolism, can also cause chest discomfort and sensations that may be mistaken for heart-related issues. Given that your mother is experiencing symptoms in colder weather, it might be worth considering whether respiratory factors are at play.

Given the complexity of the symptoms and the fact that previous evaluations have not indicated any significant cardiac issues, it may be beneficial for your mother to consult with a healthcare provider who specializes in internal medicine or pulmonology, in addition to continuing follow-ups with a cardiologist. This multidisciplinary approach can help ensure that all potential causes are explored.

In summary, while the sensation of a rapid heartbeat in the right chest area can be concerning, it is essential to consider various factors that may contribute to this experience. Continued monitoring and a thorough evaluation by appropriate specialists will be crucial in determining the underlying cause and ensuring your mother's health and well-being. If symptoms worsen or new symptoms arise, seeking immediate medical attention is advisable.
