Involuntary twitching of the right chest?

Hello, Doctor: For more than the past six months, I have been experiencing an involuntary pulsating sensation in the inner part of my right chest, similar to a heartbeat, but more rapid, which causes discomfort.
When it becomes severe, it is often accompanied by coughing, which helps alleviate the sensation.
In terms of seasonal patterns, I feel that the frequency of this occurrence is much lower in the summer, but now that we are entering autumn, the likelihood has increased again.
I visited a cardiologist in January and wore a Holter monitor for 24 hours, which showed normal results.
In June, I underwent a health check that included an electrocardiogram, which was also normal.
A chest X-ray taken was normal as well.
In October, my follow-up blood tests for hyperthyroidism were also normal.
I have been monitoring my hyperthyroidism for nearly eight years and have been off medication for six and a half years.
Although my blood test results have consistently been normal, I notice that my heart rate increases after brisk walking, climbing stairs, or exercising, and it does not return to baseline quickly, though it does not significantly affect my daily life.
However, the aforementioned issue causes me discomfort and raises concerns about whether there might be a problem with my heart or thoracic cavity.

Dr. Yang Wenda reply Pulmonology

Hello: There are two scenarios that can cause a sensation of palpitations: one is a genuinely fast heart rate, and the other is palpitations.
Your situation seems to lean towards the latter, as the electrocardiogram and 24-hour monitoring did not show significant tachycardia or arrhythmia, and thyroid function is normal.
It is normal for the heart rate to increase after climbing stairs or exercising, and a slower recovery rate may indicate a need for more physical conditioning.
Palpitations are characterized by a subjective feeling of a strong or rapid heartbeat, but monitoring your heart rate with a watch for one minute did not show an increase.
Some cases may be related to psychological stress or poor sleep quality.
You may consider consulting a traditional Chinese medicine practitioner or a mental health specialist if there are issues with adjustment or life stress.
The above suggestions are for your reference!

More Info

Unexplained right chest pulsations can be concerning, especially when accompanied by discomfort and cough. Given your medical history and the tests you've undergone, let's explore potential causes and considerations for your symptoms.

Possible Causes of Right Chest Pulsations
1. Musculoskeletal Issues: One common cause of pulsations in the chest area is related to the musculoskeletal system. Conditions such as muscle spasms or inflammation of the rib cage (costochondritis) can lead to sensations that mimic heartbeats. These symptoms can be exacerbated by physical activity or changes in posture.

2. Cardiac Factors: Although you have had normal results from your cardiac evaluations, it’s important to consider that some heart conditions may not always be detected through standard tests like ECG or X-rays. Conditions such as arrhythmias or palpitations can sometimes present as pulsations. Given your history of hyperthyroidism, it's worth noting that thyroid disorders can influence heart rate and rhythm, potentially leading to sensations of rapid heartbeats or pulsations.

3. Pulmonary Issues: Since you mentioned that the pulsations sometimes accompany coughing, it may be worthwhile to consider pulmonary causes. Conditions such as pleuritis (inflammation of the lining of the lungs) or even pulmonary hypertension can lead to discomfort and unusual sensations in the chest. However, your recent imaging studies have shown no significant abnormalities, which makes this less likely.

4. Anxiety and Stress: Psychological factors can also manifest physically. Anxiety or panic attacks can lead to sensations of palpitations or pulsations in the chest. If you find that your symptoms correlate with stress or anxiety levels, this could be a contributing factor.

5. Seasonal Variations: You noted that the symptoms seem to worsen in the fall and are less frequent in the summer. This could suggest an environmental or allergenic trigger, particularly if you have any underlying respiratory conditions that might be exacerbated by seasonal changes.

Recommendations
1. Further Evaluation: Since your symptoms persist and cause discomfort, it may be beneficial to consult with a cardiologist or a pulmonologist for a more comprehensive evaluation. This could include advanced imaging studies or a Holter monitor for longer-term heart rhythm assessment.

2. Monitoring Symptoms: Keep a detailed log of your symptoms, noting the frequency, duration, and any associated activities or triggers. This information can be invaluable for your healthcare provider in diagnosing the issue.

3. Lifestyle Modifications: Consider lifestyle factors that may influence your symptoms. Regular exercise, stress management techniques, and a balanced diet can help improve overall cardiovascular health and may alleviate some symptoms.

4. Follow-Up on Thyroid Health: Since you have a history of hyperthyroidism, regular follow-ups with your endocrinologist are essential to ensure that your thyroid levels remain stable and do not contribute to your symptoms.

5. Consider Psychological Support: If anxiety is suspected to be a factor, speaking with a mental health professional may provide strategies to manage stress and anxiety, which could, in turn, alleviate some of the physical symptoms.

Conclusion
While your previous evaluations have returned normal results, persistent symptoms warrant further investigation. It’s essential to communicate openly with your healthcare providers about your concerns and symptoms. They can help guide you through the next steps in diagnosis and management, ensuring that any underlying issues are addressed appropriately. Remember, your health and comfort are paramount, and seeking clarity on these symptoms is a crucial step in maintaining your well-being.
