Post-operative frequent urination after circumcision?
Hello, doctor.
Four weeks ago, I underwent laser circumcision due to phimosis.
Before the surgery, I did not have any issues with frequent urination, and I was masturbating an average of twice a week.
However, starting from the second week after the surgery, I began experiencing frequent urination.
I drink between 2000cc to 3000cc of water daily, and there is no pain while urinating.
Shortly after using the restroom, I feel a persistent urge to urinate, going to the bathroom 3 to 4 times between 1 AM and 3 AM.
I have a sensation of incomplete bladder emptying, needing to exert more effort to expel the remaining urine.
Occasionally, I experience general weakness and a feeling of urgency as if I am about to urinate.
I returned to the surgical clinic for a follow-up and discussed my condition with the doctor, who prescribed a 3-day course of antibiotics, but there was no improvement.
I then visited another hospital where I underwent two urinalyses, both of which were normal, and abdominal ultrasound and X-rays also showed no kidney stones.
One of the doctors mentioned that since the glans was previously covered, it may now be hypersensitive due to the adjustment, leading to frequent urination.
My current condition fluctuates.
I have a few questions for the doctor: 1.
Given my current situation, do you recommend any further examinations, such as for the prostate or bladder? 2.
It has been 28 days post-surgery, and there is one area with scar tissue that hurts slightly when touched.
The doctor has checked it and found no issues; does this mean the wound is healing? 3.
Am I currently able to engage in activities like jogging?

Hello: 1) It may be primarily observational, as the circumcision procedure is unrelated to the prostate and bladder.
2) Healing has occurred, but there may be issues with nerve and vascular recovery or complete restoration.
Wishing you good health.

Post-circumcision urinary frequency can be a distressing issue, especially when it arises after a surgical procedure that is typically not associated with such symptoms. Your situation, where urinary frequency began two weeks post-surgery, is not uncommon, and there are several factors that could be contributing to your symptoms.
Firstly, it is essential to understand that after circumcision, the glans (the head of the penis) is exposed for the first time, which can lead to increased sensitivity. This heightened sensitivity may cause a sensation of urgency or frequent urination, as the nerves in the area become accustomed to the new exposure. This phenomenon is sometimes referred to as "glans hypersensitivity." The body may take time to adjust to this change, and during this period, you might experience increased urinary frequency and urgency.
Regarding your specific questions:
1. Further Testing: Given that you have already undergone multiple urine tests, abdominal ultrasounds, and X-rays that returned normal results, it may not be immediately necessary to pursue further invasive testing like prostate or bladder examinations unless your symptoms worsen or new symptoms develop. However, if the urinary frequency persists or is accompanied by other symptoms such as pain, blood in the urine, or significant discomfort, it would be prudent to consult a urologist for a thorough evaluation. They may consider tests such as a cystoscopy or urodynamic studies to assess bladder function and rule out any underlying issues.
2. Wound Healing: The presence of a single area of fibrosis that is painful upon touch suggests that while most of the surgical site may be healing well, this specific area may still be in the process of healing or may have developed some scar tissue. It is not uncommon for surgical sites to have areas of sensitivity or discomfort for several weeks post-operation. If the pain is mild and does not worsen, it may be a normal part of the healing process. However, if you notice increased pain, swelling, or any signs of infection (such as redness or discharge), you should return to your surgeon for an evaluation.
3. Exercise: As for engaging in activities like jogging, it is generally advisable to wait until you feel comfortable and pain-free in the area of the surgery. Light activities can often be resumed after a couple of weeks, but high-impact exercises should be approached with caution. If you experience any discomfort while jogging or if your symptoms worsen, it would be wise to stop and consult your healthcare provider.
In summary, while your symptoms may be linked to the sensitivity of the glans post-circumcision, it is essential to monitor your condition closely. If symptoms persist or worsen, further evaluation may be warranted. In the meantime, consider practicing pelvic floor exercises, which can help with urinary control and may alleviate some of the urgency you are experiencing. Additionally, maintaining a balanced fluid intake and avoiding bladder irritants (like caffeine and spicy foods) can also be beneficial. Always consult with your healthcare provider before making any significant changes to your activity level or if you have concerns about your recovery.
