Primaline cream issues?
Hello, Doctor: Three years ago, I had difficulty treating frequent urination issues and eventually consulted a gynecologist who administered Premarin cream vaginally.
After a month, my discomfort disappeared.
I then switched to oral hormone therapy, which I continued for over two years before stopping the medication.
Recently, the frequent urination issue has resurfaced, and upon returning to the gynecologist, the doctor advised me to use Premarin for one month and then to start long-term supplementation of estrogen and progesterone.
Since I have two 2 cm fibroids, I need to combine this with oral medication to suppress endometrial hyperplasia.
My question is: Is it possible to use Premarin for a short term (one month) without taking hormones? I feel stressed about the prospect of long-term hormone supplementation...
Thank you, Doctor.

Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, short-term use of Primarin (for one month) only provides temporary effects.
It is recommended to continue long-term supplementation of estrogen and progesterone, with regular follow-ups with an obstetrician-gynecologist.
Wishing you good health.

Premarin cream, a form of conjugated estrogens, is often prescribed for various conditions related to estrogen deficiency, including urinary issues such as frequent urination, which can be a symptom of vaginal atrophy or other estrogen-related conditions. Your previous experience with Premarin cream, where symptoms improved significantly after a month of use, suggests that it can be effective in alleviating such symptoms in the short term.
In your case, using Premarin cream for one month without additional hormonal therapy may provide temporary relief from urinary symptoms. However, it is essential to understand that while short-term use can be beneficial, it may not address the underlying hormonal imbalance that contributes to your urinary issues. Estrogen plays a crucial role in maintaining the health of the vaginal and urinary tract tissues, and a deficiency can lead to symptoms like dryness, irritation, and urinary frequency.
Your healthcare provider's recommendation to use Premarin cream for a month before transitioning to long-term estrogen and progesterone therapy is a common approach. This strategy allows for an initial alleviation of symptoms while preparing your body for a more sustained hormonal treatment. The combination of estrogen and progesterone is particularly important in your case, given the presence of uterine fibroids, as it helps to mitigate the risk of endometrial hyperplasia, which can occur with unopposed estrogen therapy.
While the idea of long-term hormonal therapy may feel overwhelming, it is crucial to weigh the benefits against the potential risks. Hormonal treatments can significantly improve quality of life by alleviating symptoms associated with menopause or hormonal imbalances, including urinary issues, hot flashes, and vaginal dryness. Regular follow-ups with your healthcare provider can help monitor your response to treatment and make necessary adjustments.
Additionally, it is worth discussing any concerns you have about long-term hormone therapy with your doctor. They can provide insights into the safety, efficacy, and potential side effects based on your individual health profile. There are also alternative treatments and lifestyle modifications that can support urinary health, such as pelvic floor exercises, dietary changes, and the use of vaginal moisturizers or lubricants.
In summary, while short-term use of Premarin cream may help with your urinary issues, it is advisable to follow your doctor's guidance regarding long-term hormonal therapy. This approach not only addresses your immediate symptoms but also helps manage your overall hormonal health in the context of your fibroids and other considerations. Always consult your healthcare provider before making any changes to your treatment plan to ensure it aligns with your health needs and goals.
