Post-Surgery Concerns After Fistula and Hemorrhoid Surgery: A Guide - Surgery

Post-fistula surgery

Hello Doctor: I underwent a fistulotomy and mixed hemorrhoid surgery on December 28, 2018.
It has been almost four months since the procedure.
Although I can run and sit for extended periods now, I still have a few concerns: 1.
I often notice discharge a few hours after having a bowel movement in the morning, which continues until the evening (I have a history of bowel dryness, so my stools are mostly well-formed soft stools, and my bowel movements are smooth).
2.
There are frequently some small, elongated wounds near the anal opening and directly behind it that keep reappearing.
I would like to consult with you regarding these issues.
Thank you for your response.

Xiao Huang, 40~49 year old female. Ask Date: 2019/04/21

Dr. Xu Kaixi reply Surgery

1.
If the discharge is not blood-tinged or purulent, it may be intestinal mucosal secretions, and observation is sufficient.
2.
It could be a minor tear in the scar tissue from postoperative healing; applying a topical ointment to the wound is adequate.

After undergoing surgery for a fistula and mixed hemorrhoids, it is not uncommon to experience some post-operative concerns, especially in the first few months following the procedure. Your situation, where you are experiencing discharge and small wounds around the anal area, can be quite distressing, but it is important to understand that these symptoms can occur as part of the healing process.

1. Discharge After Bowel Movements: The presence of discharge several hours after a bowel movement can be attributed to a few factors. After surgery, the tissues around the anal area may still be healing, and it is possible for mucus or other secretions to be produced as part of the healing process. This is particularly true if you have a history of bowel issues, such as irritable bowel syndrome (IBS), which can lead to softer stools and increased mucus production. If the discharge is not accompanied by pain, bleeding, or foul odor, it is likely a normal part of recovery. However, if the discharge persists or worsens, it would be prudent to consult your surgeon or a gastroenterologist to rule out any complications, such as infection or an unresolved fistula.

2. Recurrent Small Wounds: The appearance of small wounds or fissures near the anal area can be a common issue after such surgeries. These may be due to several reasons, including the healing process itself, irritation from bowel movements, or even the presence of residual hemorrhoidal tissue. It is essential to maintain good hygiene in the area to prevent infection and promote healing. Gentle cleaning with warm water and avoiding harsh soaps can help. Additionally, using a barrier cream or ointment may provide relief and protect the area from irritation.

Post-Surgical Care Recommendations:
- Dietary Adjustments: Since you mentioned having soft stools, it is crucial to maintain a balanced diet rich in fiber to ensure regular bowel movements without straining. Foods high in fiber, such as fruits, vegetables, and whole grains, can help achieve this. Staying hydrated is equally important.

- Sitz Baths: Taking warm sitz baths can help soothe the anal area, promote blood flow, and aid in healing. This can be done several times a day, especially after bowel movements.

- Avoid Straining: It is vital to avoid straining during bowel movements, as this can exacerbate any existing wounds or fissures. If you find yourself straining, consider discussing stool softeners or mild laxatives with your healthcare provider.

- Follow-Up Appointments: Regular follow-ups with your surgeon are crucial. They can assess the healing process and address any concerns you may have. If you notice any signs of infection, such as increased pain, swelling, or discharge with a foul odor, seek medical attention promptly.

- Physical Activity: While you mentioned being able to run and sit for extended periods, it is essential to listen to your body. Gradually increasing your activity level can help, but be cautious of any discomfort.

In conclusion, while some discharge and minor wounds can be part of the healing process after fistula and hemorrhoid surgery, it is essential to monitor your symptoms closely. Maintaining good hygiene, dietary adjustments, and regular follow-ups with your healthcare provider will aid in your recovery. If you have any concerns or if your symptoms worsen, do not hesitate to reach out to your healthcare provider for further evaluation and management.
