Tightness in the clavicle?

Hello, Doctor.
I have post-nasal drip and mild gastroesophageal reflux disease (GERD).
The doctor mentioned that I have mild asthma.
Previously, I experienced a tight sensation around my collarbone, but it would resolve on its own even without medication.
For the past six months, I haven't felt that tightness, but since early April, I have been feeling tightness around my collarbone again.
I don't have shortness of breath, but sometimes deep breathing seems to help.
Last Friday, I visited the pulmonology department at Zhongxiao Hospital, and they said it shouldn't be a bronchial issue since I don't cough or experience shortness of breath with the tightness.
They referred me to an ENT specialist.
The ENT doctor at Zhongxiao Hospital attributed my symptoms to post-nasal drip and prescribed medication, which I have been taking, along with a bronchodilator.
I felt some improvement on Sunday and Monday, but on Tuesday, I again felt tightness in my throat and bronchial area, although it wasn't constant throughout the day.
It felt better at night.
Today, I consulted an ENT doctor at National Taiwan University and explained my symptoms of tightness in the throat and bronchial area.
He examined my throat and noted a small polyp in my pharynx, stating it was very small and that the airway was quite clear, so it shouldn't affect my breathing.
I have been scheduled for hospitalization on May 17 to have the polyp removed for further examination.
Although the doctor said it appears to be harmless, I am quite worried.
I don't smoke or drink alcohol, and I would like to ask: 1.
The doctor mentioned that only smoking, drinking, or GERD could cause polyps, is that correct? 2.
I read online that early-stage throat cancer can cause breathing difficulties.
Do my symptoms resemble that? I am already starting to worry about whether there is something wrong.
I had a gastroscopy last year that was normal, showing only mild reflux, and I had a chest X-ray on November 7 that was also normal.
I am completely confused about why I feel tightness around my collarbone and whether I should get another X-ray.

Hello: There are many causes of granulation tissue, commonly associated with infections or chronic irritation.
Conditions such as post-nasal drip or gastroesophageal reflux are factors of chronic irritation.
It is recommended to follow your physician's advice for further examination.
Wishing you safety and good health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan General Hospital, Ministry of Health and Welfare.

The symptoms you are experiencing, particularly the tightness around the collarbone and the sensation of constriction in your throat, can be concerning, especially given your history of gastroesophageal reflux disease (GERD) and mild asthma. It’s understandable to feel anxious about these sensations, particularly when they are recurrent or change in nature.
Firstly, it’s important to note that tightness around the collarbone can be attributed to various factors, including muscular tension, anxiety, or even referred sensations from other areas of the body. Given that you have a history of nasal drip and GERD, it’s plausible that these conditions could contribute to your symptoms. Postnasal drip can lead to throat irritation, which might manifest as a feeling of tightness or discomfort in the throat or upper chest area. Similarly, GERD can cause esophageal irritation, which may also contribute to sensations of tightness or discomfort in the throat and chest.

Regarding the small growth (or polyp) that was found in your throat, it’s not uncommon for such growths to occur due to chronic irritation, which can be caused by factors such as smoking, alcohol consumption, or acid reflux. While your doctor has indicated that the growth is small and unlikely to affect your breathing, it’s wise to follow through with the recommended biopsy to rule out any serious conditions.
To address your specific questions:
1. Causes of Small Growths in the Throat: While smoking and alcohol are common irritants that can lead to the development of throat polyps or growths, chronic irritation from acid reflux can also be a significant contributing factor. Other potential causes could include chronic throat infections or vocal strain, especially if you use your voice extensively.

2. Concerns About Throat Cancer: Early-stage throat cancer can present with symptoms such as a persistent sore throat, difficulty swallowing, or changes in voice. However, the presence of a small growth alone, especially in the context of your other symptoms and the assessments by your healthcare providers, does not necessarily indicate cancer. It’s important to remember that many benign conditions can present similarly.

As for the tightness you feel around your collarbone, it may not be directly related to your respiratory or gastrointestinal issues. Stress and anxiety can manifest physically, leading to muscle tension in the neck and shoulders, which might contribute to the sensation of tightness. Additionally, deep breathing exercises can sometimes alleviate feelings of tightness, as you have noted.

Given your concerns, it may be beneficial to engage in relaxation techniques, such as deep breathing exercises, yoga, or meditation, to help manage anxiety and muscle tension. Regular follow-ups with your healthcare providers are essential, and if your symptoms persist or worsen, further imaging studies, such as a chest X-ray or CT scan, may be warranted to rule out any underlying issues.

In summary, while your symptoms are understandably concerning, they could be attributed to a combination of factors including GERD, postnasal drip, and possibly anxiety-related muscle tension. Following your doctor's recommendations for further evaluation of the throat growth is crucial, and maintaining open communication with your healthcare team will help ensure that you receive appropriate care and peace of mind.
