Repeated failures in quitting smoking?
Hello Doctor, I have been smoking for 11 years, about a pack a day.
In the past four years, I have tried to quit smoking four times using medications like Chantix, patches, gum, and inhalers, but I have repeatedly failed.
The longest I managed to stay smoke-free was three months, which has left me feeling very disappointed and frustrated with myself.
I also struggle with depression.
Last week, I tried to quit smoking again and went to the pharmacy to get nicotine patches, but I'm not sure if the dosage was sufficient because I feel like the effect isn't very good.
My cravings are still strong, and I smoke about 5 to 10 cigarettes a day.
I know this isn't right, but I can't seem to overcome the pressure and desire.
I'm really disappointed in myself, and it has also disappointed my wife.
Doctor, I would like to ask you...
what other methods are available? I truly feel helpless.

Dr. Chen Houliang reply Smoking Cessation
Dear Mr.
R: Depression can often lead to feelings of pessimism and helplessness, resulting in negative thoughts.
You may want to discuss with your physician how effective treatment for depression can help you approach smoking cessation with a more positive attitude.
In the past, we have found that when emotional well-being improves, cravings for cigarettes often significantly decrease.
With determination, quitting smoking is definitely achievable.
As for smoking cessation medications, they are merely an adjunctive method; there are many diverse approaches to quitting smoking, including relying on personal willpower, support from friends and family, recommendations and encouragement from healthcare teams, or even assistance from smoking cessation hotlines.
Additionally, you can seek out a nearby clinic to discuss smoking cessation with a physician to find the method that works best for you.
Thank you for your letter, and I wish you success in quitting smoking.

More Info
Overcoming smoking addiction is a challenging journey, especially for someone like you who has been smoking for 11 years and has attempted to quit multiple times. It’s understandable to feel frustrated and disappointed, especially when you have tried various methods such as nicotine patches, gum, and inhalers without lasting success. However, it’s important to remember that many people go through several attempts before they successfully quit smoking. Here are some strategies that may help you achieve lasting success in overcoming your smoking addiction.
1. Understand Your Triggers
Identifying the situations, emotions, or environments that trigger your urge to smoke is crucial. Keep a smoking diary for a week, noting when you smoke, what you were doing, and how you felt. This can help you recognize patterns and develop strategies to avoid or cope with these triggers. For instance, if you tend to smoke when stressed, consider alternative stress-relief techniques such as deep breathing, meditation, or engaging in a hobby.
2. Seek Professional Help
Given your history of depression, it may be beneficial to seek professional help. A healthcare provider can offer tailored advice and support, possibly including therapy or counseling. Cognitive-behavioral therapy (CBT) has been shown to be effective in helping individuals change their smoking behaviors and manage cravings. Additionally, discussing your depression with a mental health professional can help you address underlying issues that may be contributing to your smoking.
3. Consider Medication Adjustments
If you find that the nicotine patches you are using are not effective, it may be worth discussing this with your healthcare provider. They can assess whether you are using the correct dosage or if a different medication might be more effective for you. There are also non-nicotine medications, such as bupropion (Zyban) and varenicline (Chantix), that can help reduce cravings and withdrawal symptoms.
4. Build a Support System
Having a strong support system can significantly increase your chances of quitting smoking. Share your goals with your family and friends, and consider joining a support group, either in-person or online. Connecting with others who are going through similar experiences can provide encouragement and accountability.
5. Develop Coping Strategies
Since you mentioned feeling overwhelmed by pressure and cravings, developing coping strategies is essential. Techniques such as mindfulness meditation, physical exercise, or engaging in creative activities can help distract you from cravings and reduce stress. Even simple activities like going for a walk or practicing yoga can be beneficial.
6. Gradual Reduction
If quitting cold turkey feels too daunting, consider a gradual reduction approach. Set a quit date and gradually decrease the number of cigarettes you smoke each day leading up to that date. This can help ease the transition and make the process feel less overwhelming.
7. Reward Yourself
Set up a reward system for yourself. Celebrate small milestones, such as one week or one month smoke-free, with a treat or activity that you enjoy. This can help reinforce your commitment to quitting and provide motivation during tough times.
8. Stay Committed and Patient
Quitting smoking is a process that often involves setbacks. It’s important to stay committed to your goal and be patient with yourself. If you slip up, don’t be too hard on yourself. Analyze what led to the relapse and use it as a learning experience to strengthen your resolve.
Conclusion
Overcoming smoking addiction is not easy, but with the right strategies and support, it is achievable. Remember that many people have faced similar challenges and have successfully quit smoking. By understanding your triggers, seeking professional help, and building a support system, you can increase your chances of success. Stay committed to your goal, and don’t hesitate to reach out for help when needed. Your health and well-being are worth the effort.
