Immune or allergic issues?
I would like to ask the doctor, my mother sometimes experiences swelling of the tongue for unknown reasons, which subsides after a while, then her palms swell, or her face swells.
Each time the swollen area is different.
She has visited many major hospitals, but they have not been able to determine the cause, only concluding that it is an immune dysfunction.
I would like to ask the doctor if this is considered a rare disease? Are there any treatment options available?

Dr. Gao Jiankai reply Internal Medicine
Hello, thank you for your inquiry.
It sounds like an allergic reaction, but it is necessary to see the patient and ask for a more detailed medical history to identify the issue.
I recommend taking your mother to see an allergist or immunologist.
The Changhua Hospital of the Ministry of Health and Welfare has an Allergy, Immunology, and Rheumatology department.
We care about you.

Understanding immune disorders can be quite complex, especially when symptoms are as varied and unpredictable as those described in your mother's case. The symptoms of swelling in different parts of the body, such as the tongue, hands, and face, can indeed be indicative of an underlying immune disorder, but they can also be associated with a variety of other conditions.
The phenomenon of swelling in different areas of the body, particularly when it is recurrent and not linked to a clear cause, can be associated with several conditions. One possibility is angioedema, which is a localized swelling that often occurs in the deeper layers of the skin and can affect areas like the face, lips, tongue, and extremities. Angioedema can be triggered by allergic reactions, medications, or even certain foods. In some cases, it can be idiopathic, meaning that no specific cause can be identified.
Another consideration is the possibility of an autoimmune disorder. Autoimmune diseases occur when the immune system mistakenly attacks the body's own tissues. Conditions such as systemic lupus erythematosus (SLE), rheumatoid arthritis (RA), or Sjögren's syndrome can present with symptoms of swelling, among other systemic manifestations. In these cases, the immune system's dysregulation can lead to inflammation and swelling in various parts of the body.
Your mention of "immune dysregulation" suggests that there may be an imbalance in the immune response, which could be contributing to the swelling. This could be due to a variety of factors, including genetic predisposition, environmental triggers, or infections. It is important to note that while some autoimmune diseases are relatively common, others can be quite rare, and the symptoms can overlap significantly, making diagnosis challenging.
In terms of treatment, the approach will depend on the underlying cause of the swelling. If an allergic reaction is suspected, antihistamines or corticosteroids may be prescribed to manage symptoms. For autoimmune conditions, treatment may involve immunosuppressive medications to help regulate the immune response and reduce inflammation. In some cases, lifestyle modifications, such as dietary changes or stress management techniques, may also be beneficial.
Given that your mother has seen multiple specialists without a definitive diagnosis, it may be worthwhile to consider a referral to a rheumatologist or an immunologist. These specialists can conduct more comprehensive evaluations, including blood tests for specific autoantibodies, inflammatory markers, and possibly imaging studies to assess for underlying conditions.
In conclusion, while the symptoms your mother is experiencing are concerning and warrant further investigation, they are not necessarily indicative of a rare disease. The complexity of the immune system means that many conditions can present with similar symptoms. A thorough evaluation by a specialist will be crucial in determining the appropriate diagnosis and treatment plan. It is essential to remain proactive in seeking answers and advocating for your mother's health, as early intervention can often lead to better outcomes.
