Testicular issues?

In recent years, I have noticed that my left testicle feels different from my right testicle; the sensation is not painful, so I didn't think much of it.
However, recently while sleeping, I discovered that the left testicle has a noticeable tingling sensation, which is quite different from the right.
Upon self-examination, I found a soft mass beneath the left testicle that causes pain when I apply slight pressure.
I also experience a similar aching sensation in my left abdomen.
I am considering seeking medical attention, so I would like to inquire about the potential conditions related to these symptoms and whether an X-ray is necessary.
Additionally, I have a personal history of being overweight; could this be related? Furthermore, I had a circumcision as a child and have experienced severe pain in the genital area during rough play, which I managed to endure.
I am concerned whether this could indicate serious testicular bleeding.
Thank you for your response, Doctor.

Dr. Sun Hongshun reply Urology

Hello, as you mentioned, inflammation of the epididymis, varicocele, or trauma causing bleeding are all possible.
An X-ray would not be very helpful in your case.
If you have time, you are welcome to come to our hospital to arrange for an ultrasound examination.

When it comes to testicular issues, it is essential to understand the symptoms, potential concerns, and when to seek medical help. Based on your description, it seems you are experiencing some unusual sensations in your left testicle, including a feeling of difference compared to the right, a soft mass beneath the left testicle, and associated discomfort in the left abdomen. These symptoms warrant careful consideration.

Firstly, it is important to note that any noticeable change in the testicles should not be ignored, even if there is no pain initially. The presence of a soft mass or any abnormality can indicate various conditions, some of which may require prompt medical attention. Common issues that can arise include:
1. Epididymitis: This is an inflammation of the epididymis, often caused by infection. Symptoms can include swelling, pain, and sometimes a feeling of heaviness in the affected testicle.

2. Hydrocele: This is a fluid-filled sac around a testicle that can cause swelling. While it is usually painless, it can lead to discomfort if it becomes large.

3. Varicocele: This is an enlargement of the veins within the scrotum, which can cause a feeling of heaviness or discomfort, particularly when standing or during physical activity.

4. Testicular torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. Symptoms include sudden, severe pain and swelling. This condition requires immediate medical intervention.

5. Tumors: While less common, testicular tumors can present as a lump or mass in the testicle. They may or may not be painful and can occur at any age.

Given your symptoms, particularly the soft mass and discomfort, it is advisable to seek medical evaluation. A healthcare provider will likely perform a physical examination and may recommend an ultrasound of the scrotum to assess the structures and identify any abnormalities. An ultrasound is a non-invasive imaging technique that can provide valuable information about the condition of the testicles and surrounding tissues.

Regarding your concerns about body weight, being overweight can contribute to various health issues, including hormonal imbalances and increased risk of certain conditions. However, it is not directly linked to testicular issues. Nonetheless, maintaining a healthy weight through diet and exercise can improve overall health and potentially alleviate some discomfort.

As for your childhood experiences, while past trauma can sometimes have long-term effects, it is unlikely that a previous injury would directly cause the symptoms you are currently experiencing. However, if you have concerns about any past injuries, it is worth discussing them with your healthcare provider.

In summary, it is crucial to address any changes in your testicular health promptly. While some conditions may be benign, others can be serious and require immediate attention. Do not hesitate to reach out to a healthcare professional for a thorough evaluation and appropriate management. Early intervention can lead to better outcomes and peace of mind.
