Consultation regarding the antihypertensive medications Amlodipine and Entresto?
Hello: I have high blood pressure and regularly take antihypertensive medications.
I take the following daily: 1.
Olmetec 20mg (Olmesartan) once a day, and 2.
Inderal 10mg (Propranolol) twice a day, in the morning and evening.
I would like to ask the pharmacist about the long-term effects of taking these two medications on my body.
I have also heard that long-term use of Inderal may lead to heart failure; is this true? Should I discuss changing my medication with my doctor? Thank you for taking the time to respond.
A Qian, 30~39 year old female. Ask Date: 2019/06/20
Dr. Hou Fangmin reply Pharmacology
Dear Mr.
Ah-Hsien,
The medication Olmetec that you are taking is for hypertension, and there are no interactions with Inderal.
Additionally, Inderal has multiple uses, such as for migraines, arrhythmias, and anxiety, and it is quite effective for these conditions.
Regarding the concern of whether this medication could cause heart failure, it is rarely listed as a side effect unless there is pre-existing congestive heart failure, in which case your physician will assess your situation carefully.
Medications are not prescribed lightly.
Furthermore, for patients with diabetes, Inderal can lead to hypoglycemia, which will also be evaluated by your doctor.
Wishing you good health, peace, and happiness.

When considering the long-term effects of medications like Olmetec (Olmesartan) and Inderal (Propranolol) for hypertension, it is essential to understand both the therapeutic benefits and potential side effects associated with these drugs.
Olmetec (Olmesartan) is an angiotensin II receptor blocker (ARB) commonly prescribed for hypertension. It works by relaxing blood vessels, which helps lower blood pressure. Long-term use of Olmesartan is generally well-tolerated, and many patients experience significant benefits without severe side effects. However, some potential side effects include dizziness, fatigue, and in rare cases, kidney dysfunction or elevated potassium levels (hyperkalemia). Regular monitoring of kidney function and electrolytes is advisable, especially in patients with pre-existing kidney issues or those on other medications that affect renal function.
Inderal (Propranolol) is a beta-blocker that is often used not only for hypertension but also for anxiety, migraine prevention, and certain heart conditions. While it is effective in lowering heart rate and blood pressure, long-term use can lead to side effects such as fatigue, cold extremities, and in some cases, respiratory issues, particularly in patients with asthma or chronic obstructive pulmonary disease (COPD). There is a concern regarding the potential for beta-blockers to exacerbate heart failure in patients who already have compromised heart function. However, this is not a common occurrence in patients without pre-existing heart conditions.
Regarding your concern about the possibility of Inderal causing heart failure, it is important to note that while beta-blockers can have a protective effect on the heart in certain populations, they can also lead to complications in patients with existing heart failure. If you have no history of heart issues, the risk is significantly lower. Nonetheless, it is crucial to have regular follow-ups with your healthcare provider to monitor your heart function and overall health.
If you are experiencing side effects or have concerns about the long-term use of these medications, it is advisable to discuss this with your physician. They can evaluate your specific health situation, consider any symptoms you may be experiencing, and determine whether a medication adjustment is necessary. Switching to alternative antihypertensive medications may be an option if side effects become problematic or if your blood pressure is not adequately controlled.
In summary, both Olmetec and Inderal can be effective in managing hypertension, but they come with their own set of potential side effects. Regular monitoring and open communication with your healthcare provider are key to ensuring that your treatment plan remains safe and effective. If you have any specific symptoms or concerns, do not hesitate to bring them up during your next appointment. Your health and well-being should always be the priority in managing your hypertension.
