Palpitations and heart issues?

Hello, Director.
Since it was confirmed that I have mild mitral valve prolapse, I have experienced symptoms related to autonomic nervous system dysregulation.
Initially, when the condition first developed, my daily ECG recorded around a hundred missed beats (feeling like my heart was jolted) mostly during sleep or while sitting quietly.
After starting medication—half a tablet in the morning and evening—my attending physician noted that my dosage was quite low.
The issue has significantly improved, now occurring only occasionally, sometimes even not at all in a day.
However, I recently noticed that when my body feels weaker, during weather changes, after consuming unhealthy food, or when I don't sleep well, the missed beats reappear, although not as severely as before.
It has become about 10-30 times an hour.
Of course, taking Western medication helps alleviate the symptoms.
I would like to ask you: 1.
Will this issue improve as the autonomic nervous system stabilizes? 2.
Why do these missed beats occur primarily when my heart rate slows down (during quiet moments)? 3.
Is medication the only treatment for this condition? Will it never fully resolve? Because the doctor mentioned that a hundred missed beats do not constitute a level that requires treatment.
Thank you.

Hello, that's correct.
When the autonomic nervous system is stable, it will continue to improve.
It's true that you may feel it more during quiet times, but it doesn't only occur then.
Occasional missed heartbeats, even a hundred times, may not necessarily require treatment and typically have no impact on the body.
Medication is only for symptom control to make you feel more comfortable.

Heart palpitations can be a concerning symptom for many individuals, especially when they are associated with underlying conditions such as mitral valve prolapse (MVP) or autonomic nervous system dysregulation. Understanding the causes, management strategies, and the role of medication can help alleviate concerns and improve quality of life.

Understanding Heart Palpitations
1. Causes: Heart palpitations can arise from various factors, including:
- Structural Heart Conditions: Conditions like mitral valve prolapse can lead to palpitations due to irregular heart rhythms or changes in heart function.

- Autonomic Nervous System Imbalance: The autonomic nervous system regulates heart rate and can be influenced by stress, anxiety, or physical states (like rest or sleep).

- Lifestyle Factors: Caffeine, alcohol, dehydration, and poor sleep can exacerbate palpitations.

- Medical Conditions: Conditions such as hyperthyroidism, anemia, or electrolyte imbalances can also contribute to palpitations.

2. Symptoms: Patients often describe palpitations as a feeling of the heart racing, fluttering, or skipping beats. These sensations can be more noticeable during periods of rest or quiet, as the body's awareness of heart activity increases when there are fewer distractions.

Management Strategies
1. Lifestyle Modifications:
- Diet: Avoiding stimulants like caffeine and alcohol can help reduce the frequency of palpitations. Eating a balanced diet rich in fruits, vegetables, and whole grains supports overall heart health.

- Hydration: Staying well-hydrated can prevent electrolyte imbalances that may trigger palpitations.

- Sleep Hygiene: Establishing a regular sleep schedule and creating a restful environment can improve sleep quality and reduce palpitations.

2. Medication:
- As you mentioned, your physician prescribed a low dose of medication, which has helped reduce the frequency of palpitations. Medications such as beta-blockers can be effective in managing palpitations, especially those related to anxiety or autonomic dysregulation.

- It is essential to follow your physician's recommendations regarding medication and to discuss any concerns about side effects or effectiveness.

3. Monitoring and Follow-Up: Regular follow-ups with your healthcare provider are crucial. They can monitor your symptoms, adjust medications if necessary, and provide reassurance. If palpitations worsen or new symptoms arise, further evaluation may be warranted.

Addressing Your Concerns
1. Autonomic Nervous System Stability: It is possible that as your autonomic nervous system stabilizes, you may experience fewer palpitations. Stress management techniques, such as mindfulness, yoga, or deep breathing exercises, can help regulate autonomic function and reduce symptoms.

2. Occurrence During Rest: Palpitations often occur during periods of rest because the body is more attuned to internal sensations when not engaged in physical activity. When you are active, the heart rate increases, which may mask the sensation of skipped beats.

3. Long-Term Management: While some individuals may experience chronic palpitations, many find that symptoms improve over time with appropriate management. It is not uncommon for palpitations to be benign and not require aggressive treatment. Your physician's assessment that your palpitations do not warrant treatment is reassuring, but ongoing communication with your healthcare provider is essential.

Conclusion
In summary, heart palpitations can be a multifaceted issue influenced by various factors, including structural heart conditions and autonomic regulation. Management strategies include lifestyle modifications, medication, and regular monitoring. It is important to maintain open communication with your healthcare provider to address any concerns and ensure that your treatment plan aligns with your needs. If symptoms persist or worsen, further evaluation may be necessary to rule out any underlying conditions. Remember, many individuals with similar experiences find relief and improved quality of life through appropriate management and support.
