External hemorrhoids become hardened?
Hello Dr.
Ke: I started feeling pain and swelling around my anus around August 5th, and then I noticed a small protrusion at the anal opening.
That same day, I began taking warm sitz baths two to three times a day.
However, by August 12th, the pain started to increase, and the external hemorrhoid felt harder.
I read online that it can become hard in 2 to 3 days, but is it normal for it to still be painful and hard after a week? Can a thrombosed external hemorrhoid resolve on its own after warm sitz baths? I feel confused by the mixed information online...
Thank you in advance for your clarification and response.

Answer: Thrombosed external hemorrhoids can indeed be self-resorbed after warm sitz baths; however, the size of the hemorrhoid will affect the absorption time.
Small thromboses typically take about 1-2 weeks to resolve, while larger thromboses may require 3-4 weeks for complete absorption.
In severe cases, surgical excision may be necessary.
Additionally, maintaining good bowel habits is crucial; if constipation persists during this period, the external hemorrhoid may not improve easily.

Hello! Thank you for your question regarding external hemorrhoids, particularly concerning the hardening and pain associated with them. Let's delve into the causes, symptoms, and remedies for this condition.
Understanding External Hemorrhoids:
External hemorrhoids are swollen veins located around the anus. They can become painful, especially when a blood clot forms inside them, leading to a condition known as thrombosed external hemorrhoids. This can occur due to increased pressure in the rectal area, often caused by straining during bowel movements, prolonged sitting, or heavy lifting.
Causes of Hardening and Pain:
1. Thrombosis: When a blood clot forms in an external hemorrhoid, it can cause significant pain and lead to the hardening of the hemorrhoid. This is often accompanied by swelling and inflammation.
2. Inflammation: The body’s inflammatory response to the irritation or injury of the hemorrhoidal tissue can also contribute to the hardness and discomfort.
3. Infection: In some cases, an infection can develop, leading to increased pain and swelling.
4. Straining: Activities that increase abdominal pressure, such as heavy lifting or straining during bowel movements, can exacerbate the condition.
Symptoms:
- Pain or discomfort around the anus, especially during bowel movements.
- A hard lump near the anus, which may be sensitive to touch.
- Swelling and inflammation in the affected area.
- Possible bleeding during bowel movements.
Remedies and Treatment Options:
1. Warm Sitz Baths: As you have been doing, taking warm sitz baths several times a day can help alleviate discomfort and promote blood flow to the area, which may aid in healing.
2. Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or acetaminophen can help manage pain and reduce inflammation.
3. Topical Treatments: Over-the-counter creams or ointments specifically designed for hemorrhoids can provide relief from itching and discomfort.
4. Dietary Changes: Increasing fiber intake can help soften stools and reduce straining during bowel movements. Staying hydrated is also crucial.
5. Avoid Straining: Try to avoid straining during bowel movements. If you find it difficult to pass stools, consider using a stool softener.
6. Medical Intervention: If the pain persists or worsens, or if you notice significant bleeding, it is essential to consult a healthcare professional. They may recommend procedures such as rubber band ligation, sclerotherapy, or, in severe cases, surgical removal of the hemorrhoid.
Will It Resolve on Its Own?
In many cases, thrombosed external hemorrhoids can resolve on their own, especially with conservative treatment like sitz baths and pain relief. However, the hardening and pain you are experiencing may indicate that the hemorrhoid is thrombosed, and while it may eventually soften, it could take time. If the symptoms do not improve within a week or if they worsen, it is advisable to seek medical attention.
Conclusion:
It is understandable to feel confused with the varying information available online. The key is to monitor your symptoms and seek medical advice if necessary. Remember that while home remedies can be effective, professional evaluation is crucial if you experience severe pain, prolonged symptoms, or any concerning changes. Take care, and I hope you find relief soon!
