Glans issues
Hello, Doctor Du.
Last Saturday night, while I was sleeping, I experienced an erection in a semi-conscious state.
While sleeping on my side, I felt that the glans was being obstructed by my underwear, and I exerted force to achieve an erection.
At that moment, I felt a sudden pain at the tip of the glans, but I continued to sleep.
When I woke up in the morning and walked, I noticed that there was pain at the tip of the glans when it rubbed against my underwear.
There was no redness or swelling.
Pressing on the tip of the glans with my hand does not cause pain, but if I apply slight pressure with my fingertip and slide it along the tip of the glans, it causes the same pain as when it rubs against my underwear (approximately near the upper left side, which used to be the urethral opening).
Could you please tell me what this might be?

Hello: If there is external pressure on the genital area and pain occurs afterward, it is likely a minor injury.
If there are no bruising or bleeding, you can monitor the situation.
If discomfort persists, seek medical attention.
Wishing you good health.

More Info
Understanding pain in the glans (the head of the penis) can be concerning, especially when it arises suddenly and is associated with specific activities, such as sleeping in a certain position or experiencing an erection. Based on your description, it seems that the pain occurred after a moment of forceful erection while sleeping, leading to discomfort when the glans rubs against your underwear.
Possible Causes of Pain in the Glans
1. Mechanical Irritation: The most likely cause of your pain is mechanical irritation. When the penis is erect, and there is friction against clothing, it can lead to temporary discomfort or pain. This is especially true if the erection is forceful, as you described. The skin on the glans is sensitive, and any sudden or excessive pressure can cause pain.
2. Nerve Sensitivity: The glans is rich in nerve endings, making it particularly sensitive. If there was any unusual pressure or friction during the night, it could have temporarily heightened the sensitivity of the area, leading to pain upon contact.
3. Injury or Trauma: Although you mentioned there is no redness or swelling, it’s still possible that there was a minor injury to the sensitive tissues of the glans. This could happen during an erection if the penis is pressed against a hard surface or if there was an awkward position during sleep.
4. Infection or Inflammation: While less likely given the absence of redness or swelling, infections (such as balanitis) or inflammation can cause pain in the glans. These conditions are usually accompanied by other symptoms, such as discharge, itching, or swelling.
5. Urethral Issues: Since you mentioned that the pain is near the urethral opening, it’s worth considering that there could be an issue related to the urethra, such as a mild irritation or inflammation. This could be due to various factors, including urinary tract infections or sexually transmitted infections, although these typically present with additional symptoms.
Recommended Actions
1. Avoid Irritation: For now, try to avoid tight clothing and any activities that may cause further irritation to the area. Opt for loose-fitting underwear and pants to minimize friction.
2. Monitor Symptoms: Keep an eye on the pain. If it persists, worsens, or if you develop additional symptoms (such as swelling, redness, discharge, or fever), it would be prudent to seek medical attention.
3. Pain Management: Over-the-counter pain relief, such as acetaminophen or ibuprofen, can help manage discomfort if needed. However, avoid excessive use of pain relievers without consulting a healthcare provider.
4. Consult a Urologist: If the pain continues for more than a few days or if you have any concerns, it would be wise to consult a urologist. They can perform a physical examination and may recommend tests to rule out any underlying conditions.
5. Sexual Health Considerations: If you are sexually active, consider discussing any recent changes in sexual activity or practices with your healthcare provider, as this could provide additional context for your symptoms.
Conclusion
In summary, the pain you are experiencing in the glans is likely due to mechanical irritation from friction during an erection while sleeping. While this is often a benign issue, monitoring your symptoms and consulting a healthcare professional if the pain persists or worsens is important. Maintaining good hygiene and being mindful of any changes in your body will also help in managing your urological health.
