Regarding steroids and erectile phenomena?
Hello, doctor.
I am 23 years old, 190 cm tall, and weigh 73 kg.
I have not engaged in any sexual activity.
When I am not erect, I have phimosis, and my foreskin is quite loose.
When I am erect, I can fully expose the glans (I need to pull it back manually to fully expose it), and when the foreskin retracts to just below the corona, it feels tight, but I do not experience pain—just a noticeable tight sensation.
However, I can masturbate normally.
I would like to loosen the foreskin opening a bit so that it does not feel tight when erect.
I have heard that using a steroid ointment can help loosen the foreskin.
I have read a lot but am unsure which type of steroid ointment to use.
Could you please advise me on which steroid ointment is suitable for the foreskin?
Additionally, during my development in high school until now, the glans has been blue on the top and red on the bottom, gradually becoming purple with spots towards the corona.
When erect, the purple part at the corona becomes very prominent as the glans swells.
Is this a normal phenomenon of the glans becoming engorged during an erection, or is it related to the tightness of the foreskin? Since I do not feel pain when erect, just a tight sensation, could it be that the color of the glans is still maturing, and therefore the above phenomena are normal?

Hello: This platform cannot provide medical advice; you should seek medical attention for a prescription.
Do not have overly high expectations, as significant effects may not be seen in a short period, and there are many factors that can influence the outcome.
During an erection, the purple area of the corona becomes very prominent as the glans enlarges.
If it is too tight, the purple area may be larger, which is often related to individual constitution.
It is also recommended to seek medical attention together.
Wishing you good health.

More Info
When it comes to the use of topical steroid creams for conditions such as phimosis and related erection issues, it's important to understand both the benefits and potential risks associated with their use. Phimosis is a condition where the foreskin cannot be easily retracted over the glans (the head of the penis), which can lead to discomfort during erections or sexual activity.
Topical steroids, such as betamethasone or mometasone, can be effective in treating phimosis by reducing inflammation and promoting skin elasticity. The application of these creams can help soften the skin of the foreskin, making it easier to retract. However, it is crucial to use these medications under the guidance of a healthcare professional to avoid potential side effects.
Recommended Use of Steroid Creams
1. Consultation with a Healthcare Provider: Before starting any treatment, it is essential to consult with a urologist or dermatologist. They can assess your specific situation and recommend the appropriate steroid cream and dosage.
2. Application: Typically, a thin layer of the steroid cream is applied to the affected area (the foreskin) once or twice daily. It is important to follow the prescribed regimen and not to overuse the medication, as prolonged use can lead to skin thinning and other complications.
3. Duration of Treatment: Treatment duration can vary, but many healthcare providers recommend using the steroid cream for a few weeks, followed by a reassessment. If significant improvement is noted, the frequency of application may be reduced.
Understanding the Color Changes and Sensations
Regarding the color changes you described on the glans, it is not uncommon for the glans to appear different in color due to various factors, including blood flow during erections. The purple coloration you observe during erections is likely due to increased blood flow, which is a normal physiological response.
However, if you notice persistent discoloration, pain, or any other concerning symptoms, it is advisable to seek medical attention. These could be signs of underlying conditions that may require further evaluation.
Normal Development and Maturity
The changes in color and sensitivity you are experiencing may also be part of the normal maturation process of the genital area. The skin of the glans and foreskin can undergo changes during puberty and into early adulthood. As you continue to mature, these characteristics may evolve.
Conclusion
In summary, using topical steroids can be an effective way to manage phimosis, but it should be done under medical supervision. The sensations and color changes you are experiencing during erections are likely normal, but if you have any concerns or if symptoms worsen, do not hesitate to reach out to a healthcare professional for further evaluation and guidance. Your health and comfort are paramount, and a tailored approach to your situation will yield the best outcomes.
