Managing Post-Surgery Pain and Diarrhea After Hemorrhoidectomy - Surgery

Postoperative pain

Hello Doctor, I underwent a complete excision of internal and external hemorrhoids on October 11th, and today is the fifth day post-surgery.
The procedure was performed using an energy device.
For the first three days, I took a stool softener with each meal, but I still couldn't have a bowel movement.
Therefore, on the afternoon of the third day, I took a packet of Chinese herbal medicine containing rhubarb, and before bed, I increased the dosage of my Western stool softener by two pills.
On the morning of the fourth day, I finally had a bowel movement (diarrhea with a small amount).
I’m not sure if it coincided with the onset of my menstrual period.
After successfully having a bowel movement on the fourth morning, I stopped all stool softeners, but I have continued to experience diarrhea for the past two days.
Whenever I feel the urge to go, I barely make it to the bathroom in time, and I experience persistent cramping after bowel movements.
Warm sitz baths and pain medications do not completely alleviate the pain.
After cleaning the affected area, I still notice some fecal leakage shortly after.
I am very concerned about the risk of infection at the surgical site.
Doctor, should I temporarily stop drinking and eating to resolve the diarrhea issue?

Dr. Ke Fangxu reply Surgery

After hemorrhoid surgery, it is crucial to avoid both constipation and diarrhea, as either condition can affect wound healing and cause discomfort.
Regardless of the surgical method used, doctors typically prescribe stool softeners postoperatively.
The combination of Western and traditional medicine may indeed lead to diarrhea.
It is recommended to increase your intake of dietary fiber and fluids while temporarily discontinuing stool softeners.
Pain and anal control issues should improve within 1 to 2 weeks.
If there is no improvement, you should consult your surgeon for further evaluation and treatment.

After undergoing a hemorrhoidectomy, it is not uncommon to experience a range of post-surgical symptoms, including pain and gastrointestinal issues such as diarrhea. Your situation, where you have had a successful bowel movement but are now experiencing diarrhea and persistent pain, raises several important considerations regarding post-operative care and management.

Firstly, it is essential to understand that after a hemorrhoidectomy, the surgical site can be quite sensitive, and the body may react in various ways as it heals. The use of softeners and laxatives, while necessary to prevent straining during bowel movements, can sometimes lead to loose stools or diarrhea, especially if taken in excess. In your case, it seems that the combination of herbal and pharmaceutical laxatives may have contributed to your current symptoms.

Regarding your concern about stopping food and water intake to manage diarrhea, it is generally not advisable to completely halt hydration or nutrition unless specifically instructed by a healthcare provider. Dehydration can occur quickly, especially if you are experiencing diarrhea, and it is crucial to maintain adequate fluid intake. Instead, consider adjusting your diet to include bland foods that are easier on the digestive system, such as bananas, rice, applesauce, and toast (often referred to as the BRAT diet). This can help firm up stools without exacerbating the pain or discomfort associated with your surgical site.

The pain you are experiencing after bowel movements could be attributed to several factors, including irritation of the surgical site, muscle spasms, or even the presence of residual stool that may not have been expelled completely. It is vital to continue with warm sitz baths, as they can provide relief and promote healing. If over-the-counter pain medications are not providing sufficient relief, it may be worth discussing alternative pain management strategies with your physician.

Additionally, the concern about potential infection due to the presence of stool around the surgical site is valid. Maintaining cleanliness is crucial, and if you notice any signs of infection—such as increased redness, swelling, warmth, or discharge from the surgical site, or if you develop a fever—it is imperative to seek medical attention promptly.

In summary, here are some recommendations for managing your post-surgery symptoms:
1. Dietary Adjustments: Avoid excessive use of laxatives. Instead, focus on a bland diet that can help regulate your bowel movements. Stay hydrated with clear fluids.

2. Pain Management: Continue with warm sitz baths and consider discussing pain management options with your healthcare provider if over-the-counter medications are ineffective.

3. Monitor for Infection: Keep an eye on your surgical site for any signs of infection and maintain cleanliness to prevent complications.

4. Follow-Up Care: If symptoms persist or worsen, do not hesitate to contact your healthcare provider for further evaluation and management. It may be necessary to adjust your treatment plan based on your ongoing symptoms.

5. Avoid Straining: When you do have a bowel movement, try to relax and avoid straining, as this can exacerbate pain and delay healing.

Your recovery is important, and addressing these issues with your healthcare provider will help ensure a smoother healing process.
